- Statement of Work Patient Satisfaction Survey Tool & Professional Coaching Services Contract Base Year plus Four Option Years The Department of Veterans Affairs Minneapolis VA Health Care System (MVAHCS) patient satisfaction survey, data reporting and professional coaching services that does not breach our security services. The survey software must be easy for patients to navigate without or with minimal assistance. There must be no interface with the MVAHCS s information technology infrastructure and minimal to no maintenance by MVAHCS staff. The offeror provides the onsite customer support. In addition, the vendor must offer professional services including, but not limited to, the following: developing and deploying new surveys and themes; updating existing surveys and themes; training; and coaching/consulting services. Depending upon the level of professional services and amount of time needed for the vendor to complete the MVAHCS s requests for changes, an invoice may be submitted by the vendor for payment for significant survey services rendered by the vendor during the term of this contract. The MVAHCS staff must have the option of downloading the data from any of the aforementioned devices or having it sent via email by the vendor. The areas needing this service at the MVAHCS are: Inpatient Units (10 areas) Extended Care & Rehab PSL Unit 1D Unit 1E Unit 1F Unit 4J Primary Specialty Medicine (PSM) PSL Unit 3E Unit 3F Unit 3K Unit 3L Specialty Care PSL Unit 2L Spinal Cord Injury PSL Unit SCI Outpatient Clinics (5 areas) used on quarterly basis or as requested in various areas at MVAHCS and off site clinic locations example is for following areas Primary Specialty Medicine (PSM) PSL Surgical Specialty Care (SSC) PSL Extended Care & Rehab (EC&R) PSL Spinal Cord Injury (SCI/D) PSL Mental Health (MH) PSL Other areas i.e. Imaging SL, Pharmacy, off site CBOCs, etc. The above items will be awarded as an integrated service solution not on an individual line item basis. Item Descriptions Data Collection: Six handheld cellular devices for data collection (purchased). This includes all assistant cellular services i.e. mifi equipment (which I will need at least 5 out of 6 because two handheld devices will be sharing 1 mifi) Patient Survey Services: Hosting, Data, Operational Services, Reporting, Software License, and Coaching for the following scope of services for up to 15 areas to survey at any given time. Inpatient Units: 1D, 1E, 1F, 2L, 3E, 3F, 3K, 3L, 4J, SCI via handheld devices used by volunteers and/or staff Outpatient Clinics (PSM PSL, EC&R PSL, MH PSL, SSC PSL, Imaging SL, etc.) Outpatient Clinics/Other Areas: Primary Specialty Medicine (PSM) PSL, Extended Care & Rehabilitation (EC&R) PSL, Mental Health (MH) PSL, Surgical Specialty Care (SSC) PSL, Imaging SL, etc. via handheld devices can be moved to various clinics and other areas (i.e. Main Flag Atrium, Pharmacy, off site CBOCs, etc.) Data Services: Cellular data services (mifi) to be used for purpose of transmitting data to vendor that provides weekly, monthly and quarterly results or as requested. This necessary hardware and associated cellular data service facilitates cellular data transfer from the PCs to facilitate data hosting and report distribution. This is to include the following features: Intuitive data collection solution to include: Multiple collection options to work with single and multi-purpose computers: Must include both client and web applications. Store-and-forward interface for mobile and kiosk applications. 32-bit encryption of data. Automatically connect and send/receive when an internet connection is available. Branded with the Minneapolis VA HCS logo and colors Password-protection to prevent unauthorized access/use of other applications Hosted information services compliant with industry standards and best practices. Ability to modify questions, response options, reporting, and alerting within 2 business days. Ability to initiate email and pager alerts based on selected response options. Support for complex branching logic. Ability to drill-down to specific clinicians for individual locations and services. Automated information distribution to include: Stakeholder-specific graphical, statistical and text-based reporting to be provided via email at any frequency/timing desired. Specific reports that reinforce great care providers/positive practices; highlight individual, team and departmental opportunities for improvement and provide ranked results by clinical group. On-site quarterly performance improvement consultations by an individual with a minimum of 5 years patient experience measurement and improvement experience; include: Review of results Recommended changes in the design of the Assessment Program Recommended changes in the design and distribution of reporting Assessment Program Implementation services and planning tool set to include: Vetted question library and questionnaire development guide. Active involvement and guidance in developing questions to meet objectives for each location (department/unit). Best practice information use planning and consultation. Expertise and training to efficiently and effectively administer with existing staff while collecting a high and unbiased volume of responses. On-site training. Report design formatting and automation of report distribution. Professional Services: Professional services to update questions, modify reports, and provide coaching assistance and guidance. Budgeting 40 hours per year provides latitude to make changes to surveys weekly/month/quarterly as needed reports, as well as coaching key stakeholders. Maintenance/Customer Support & Professional Coaching Services to include hardware support and time associated with updating assessments and report distribution and professional coaching as directed by the Minneapolis VA HCS. Session Audience Frequency Total Number of Sessions by Contract Year Executive Performance Reviews Executive Team Quarterly 4 Performance Review and Improvement Prioritization Session for: Inpatient Nurse Managers and other Key Stakeholders Monthly 12 Performance Review and Improvement Session for: Extended Care Nurse Managers and other Key Stakeholders Quarterly 4 Performance Review and Improvement Session for: Rotating Outpatient Service Line Leader, Clinic Manager and other Key Stakeholders Quarterly 4 Performance Review and Improvement Session for: SCI (Spinal Cord Injury) Service Line Leader, Clinic Manager and other Key Stakeholders Quarterly 4 Professional Services/Modify Surveys & Reports, etc. All Stakeholders As needed 12 Total Hours 40 Transmission Devices: At least 7 mifis for the above 8 handheld devices included in the purchase - no additional charge.
