SF 30 Continuation Page 36C25719Q1416 - 0001 Question and Answers Can you please provide the type of IONM services is required and total number of cases per year? If this a 4 year contract? VA Reply - The Contractor shall provide Neuromonitoring Equipment and Technician Services for surgical procedures at North Texas Veterans Health Care System (VANTHCS), 4500 South Lancaster Road, Dallas, Texas 75216 on an as needed basis. Services shall include intraoperative, preoperative, postoperative neurophysiologic monitoring services and evoked potential testing for neurosurgery patients. Services are to be conducted in the operating room and associated areas of the Dallas VA Medical Center on a per case basis as determined by the Chief of Surgical Service and Chief of Neurosurgery. VA Reply - Please refer to B.2 Schedule of Supplies and Services within the Combined Synopsis Solicitation for the Period of Performance. Is this a new services request for this VA facility? VA Reply - This is not a new requirement.   Services of this nature have been rendered at this facility in the past. What are the Page Constraints and format requirements? VA Reply Please refer to 52.212-1 Instructions for those page limits. PWS is not cleared on Key Personnel / or Emergency Substitution. No requirements or details have been provided can the Government please provide this information? VA Reply - The PWS states requirements in general terms of what (result) is to be done, rather than how (method) it is done. The PWS gives the contractor maximum flexibility to devise the best method to accomplish the required result. If Ospreytsi feels that key personnel and emergency substitution should be included in its approach to meeting the requirement it should propose such a method.   VA Reply Please also refer to 52.212-1 Instructions and 52.212-2. These sections relate what the government wants and how it shall evaluate received proposals. Can you please advise what are the past performance requirements? The solicitation in no longer available at FBO under Solicitation Number: 36C25719Q1416 VA Reply: The VA has verified that 36C25719Q1416 has been posted to FBO. The verification was completed by reviewing 36C25719Q1416 on FBO as well as calling the Federal Service Desk to assure that it had not dropped out of the system.   Please review 36C25719Q1416 on FBO to obtain the details of this requirement for proposal development purposes. Price Cost Schedule - Line Items: 0002 Hourly Rate for Each Additional Hour of Surgery Beyond Initial 7 Hours. Can you please clarify if there is only 7 QTY Surgeries that will go over 7hs? VA Reply: The schedule request the following - Hourly Rate for Each Additional Hour of Surgery Beyond Initial 7 Hours. This CLIN 0002 request the hourly rate for each additional hour of surgery beyond the initial 7 hours. Can you please provide a case mix which it required for Neuromonitoring Neuro Surgery? VA Reply: The contractor must be prepared for a broad case mix within the neurosurgery field.   The VA cannot predict what surgeries will arise but it anticipates that it will have a broad mix of surgeries within the neuro surgery field. There is no requirement in the PWS referencing disposables and necessary electrodes for Neuromonitoring. Will the VANTHCS be providing electrodes and necessary disposal for each procedure? VA Reply:   The contractor must provide all equipment. Are Appendices or attachments excluded from the page count? VA Reply: Appendices are excluded from the page count. Attachments expounding upon or adding to your proposal covering those evaluation items in 52.212-1 and 52.212-2 shall be covered by the page count.
