- THIS IS A SOURCES SOUGHT NOTICE ONLY. This notice does NOT constitute a request for proposal, request for quote, or invitation for bid. No formal solicitation document exists currently. Market Research is being conducted to Service-Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), Small Business, Small Disadvantaged Business, 8(a), Woman Owned Small Business or HUBZone interests. The Department of Veterans Affairs Medical Center (VAMC) is trying to determine whether to set-aside an acquisition for Wheelchair Van Transportation Service for the Salisbury VAMC located at 1601 Brenner Avenue, Salisbury NC 28144. We are seeking wheelchair van transportation services for the eligible veterans located in the Salisbury VA Healthcare System. We are seeking a contractor to provide the following wheelchair van transportation services. Wheelchair Van Transportation Services: VAHCS Salisbury The Department Veterans Affairs, W.G. Bill Hefner VA Medical Center (VAMC) located in Salisbury, North Carolina has a requirement to provide Wheelchair Van transportation for eligible Veterans located in the Salisbury VA Health Care System (SVAHCS) catchment area of North Carolina and occasional trips to other VAMC s outside of SVAHCS catchment area in North Carolina for special care and treatment not provided by this VAMC. The services shall be provided twenty-four (24) hours a day, seven (7) days a week, to include Federal holidays. The Contractor shall furnish all management, supervision, labor, materials, equipment, specified supplies, and resources necessary to provide Wheelchair Van transportation for wheelchair bound and to other eligible special need Veterans. This includes one way and/or round-trip transportation services on an as needed basis from the Veterans place of residence to the designated facility within the SVAHCS or private facilities in the community. Any eligible Veteran, who requires medical services at multiple facilities on the same day, will be picked at their residence and will be transported to location of each medical appointment and returned back to their place of residence. The NAICS 485999 is applicable to this acquisition; with a size standard of $15.0 Million. If your company is interested in performing these services, please notify our office of your intent. In response, please include the following: A positive statement of your intent to submit a quote as a prime contractor (capability statement) If any teaming or subcontracting possibilities are contemplated, please provide subcontractor's name and breakdown of labor and its' performance cost Evidence of experience in work similar in type and scope to include contract numbers, project titles, award dollar amounts, points of contact and telephone numbers Past performance references with points of contact and phone numbers Provide FSS/GSA Contract# if applicable. Identify whether the firm falls into any of the socioeconomic categories listed above Your response is required by: Wednesday, October 10, 2019 by 10:00 AM EST All capable Service-Disabled Veteran Small Businesses are encouraged to respond.
