Description
This Request for Information is issued to solicit market research responses from industry to assist the Government with the analysis of a Government requirement, market sources availabilities and potential sources, including type of acquisition set-aside, if any, that may be suitable. This is not a request for quote. A. This is an acquisition for Services, to include, but not limited to: Government requirement: Reusable Sharps/Medical Waste Containers beginning December 1, 2019 Removable Reusable Puncture-Proof Pre-Assembled Leak-Proof With Overfill Protection Lockable/Tamper-Proof Sight/View Port Large Access Opening But must include restricted access to prevent hand-access (hand sizes to include children (infant and larger) and up to large adult sizes) while allowing for intended use disposal of medical sharps items (e.g. needles, syringes, scalpels) Multiple sizes from 2-gallon to 18-gallon capacities (2, 4, 8, 16, 17 gallon in all areas patient wards, clinics, operating rooms, hemodialysis rooms, etc.) Able to be sanitized and/or sterilized One-hand/One-step disposal operation mechanism Comparable with/to medical industry standards of characteristics above Mobile carts and bulk transport carts (each type with wheels) Government requirement: Disposal of Existing Sharps Medical Waste Container Types Above, to include, but not limited to: Current Quantities Approximately 310 wall-mounted containers Approximately 4 counter/table-located containers Approximately 3 mobile blood-draw cart locations (laboratory) Existing containers to be disposed of (in accordance with all applicable federal, state and local laws, regulations and industry standards government such disposal, including Dept. of Transportation and Environmental Protection Agency, Occupational Health and Safety Administration, Marion VA Medical Center Hospital and Safety Regulations and others where applicable), and replaced with new (empty like-size item) Existing container support or mounting brackets may require disposal and replacement when required to support the new containers due to size or other mounting requirements, are to be located in the same location as the removed bracket, using existing mounting holes to the extent possible and capable of holding any of the above listed standard size containers and in accordance with industry standard ergonomic practices. Manifests of containers disposed of (destruction manifests) quantities, sizes, gross weights, replacement tare weights information documenting all containers/contents transported from Marion VA Medical Center. Maintaining wheeled carts as required to be remain clean and serviceable. Government requirement: Frequency of services: Weekly. Normal business days Monday through Friday. Normal business hours 7:30am through 4:30pm, Central Time (Savings Time and Standard Time, as adjusted). Weekend services: No National Holiday services (including dates declared as a National Holidays by the President of the United States): No Chemo and Pathological Waste: No Government requirement: Locations of services: Marion VA Medical Center, 2401 W. Main St., Marion, Illinois 62959 Buildings 1, 2, 38, 43, 81, 82, 83. Floors/Rooms multiple, map(s) or locator information to be issued with later Request for Quotation Buildings, Floors and Rooms are subject to change depending on medical center requirements B. Capabilities and Socio-economic Category inquiry: Is your firm a small business? (any category) Is your firm a Veteran-Owned Business, with CVE verification? VOSB? SDVOSB? Is your VOSB/SDVOSB a small business within the small business threshold for NAICS 562112 ($41.5mil)? Does your firm regularly and routinely provide the services, including all materials, of the type and quality as described above? Does your firm perform the services without the use of subcontractors? If subcontractors are used, what percentage of the work does your firm perform versus what percentage of the work does your subcontractor perform? Does your firm have access to, or otherwise provide the types of containers and carts described above? Does your firm have the capability to replace brackets where necessary in the course of replacing existing containers and brackets and accepting any of the standard size containers? Does your firm have all necessary licenses, permits, insurance and trained personnel and access to materials and industry-specific disposal & destruction methods sufficient to perform the services described above, including the indicated quantities, types of services and services frequencies? Will your firm be able to start services on December 1st, 2019 (required), with the first day of services being December 1st, 2019? Does your firm anticipate you would submit a competitive price quote for these services in response to a solicitation to be issued in support of the Marion VA Medical Center? Does your firm have the capacity to enter into an option year contract, e.g. a base 1-year period and four 1-year optional periods? Can your firm provide detailed invoices, to include quantities of services (including container counts, locations, sizes) performed, dates of performances and itemized line item pricing? Can your firm provide disposal manifests as required for this type of services, and provide the manifests weekly? Can your firm perform the above described services in multiple buildings, with more than 1 floor and with limited or restrictive elevator access? C.
