Description
STATEMENT OF WORK AND CRITERIA FOR TITLE: Hot water heater on CLC2(Community Living Center) BACKGROUND INFORMATION: Hot water heater on roof of CLC2 requires installation of graphics and alarms. Does not have building automation system with graphic control. The purpose of this contract is to have a Contractor to furnish all labor, materials, supplies, tools, and install graphics and sensors alarm notifications from hot water heater located on roof of CLC2(Community living center) to communicate to siemens computer located in graphic control room 1D-16A this will allow operators to monitor hot water heater as per VHA directive 1810 safety directives and OSHA standards. All service procedures detailed below will be performed on listed equipment. Hot Water Heater Model SERIAL SCAQMD-Model-8TPL 1000LHE250A-TPL F004852 1.Contractor will provide all electrical components. 2. VA Plant operators will tag out hot water heater. 3.Contractor will also tag-and lock out hot water heater. 4.Contractor will check in and check out daily with plant operators. 5.Contractor will not start work till after 8am due to patients living in CLC. 6.Contractors will start when all parts for job have been acquire. 7.Contractor will inform of progress of work daily to graphic control. 8.Contractor will run electrical conduit install relays and sensors. 9.Contractor will terminate wiring at siemens panel on CLC2 roof. 10.Contractor will provide hot water temperature input 11.Contractor will provide status input of the hot water circulation pump. 12.Contractor will provide alarm input for the hot water heater. 13.Contractor will build graphics at siemens front end computer located in 1D-16A. 11.Contractor will ensure alarm notification are activated. 12.Contractor will provide with diagram of new wiring. 13.Contractor will properly clean working area and dispose of parts and left over trash in appropriate disposal site. 15.VA operators will inspect area before contractors depart work area. PERIOD OF PERFORMANCE: 2 weeks PLACE OF PERFORMANCE: VA Loma Linda 11201 Benton St Loma Linda, CA 92357 LICENSING REQUIREMENTS: Offeror must possess a 30-hour Occupational Safety and Health Administration (OSHA) safety card. EPA license C-10 license The contractor will provide all required licenses, certifications, permits, documents, as required and requested. The contractor will provide all required, qualified, license, and certified labor, technicians, equipment, materials, tools, transport, supervision, safety requirements, personal protective equipment (e.g., safety boots, safety glasses/goggles, gloves, protective wear, masks, etc.). SPECIFIC REQUIREMENTS: Schedule with Contracting Officer Representative (COR) or Designee five (5) business days in advance prior to start of work. Prior to start of job, all personnel shall have a valid Loma Linda PIV badge Prior to working, ALL energy sources (electrical, water, etc.) shall be locked and tagged out, and upon completion of work, will be re-energized and verified. Prior to work each day, workers or a designated lead or supervisor are/is to check in with the Graphics Control in the plant. Contractor shall conform to the following standards, policies, regulations, governing agency, and any other local, state, or Federal guidelines that are not listed here OSHA Regulations, www.osha.gov NFPA, www.nfpa.org
