- This modification is to delete evaluation factors in the Pre-Solicitation and replace them with the ones below and insert Past Performance Questionnaire. Evaluation Factors: SELECTION CRITERIA: Selection will be based on the following criteria: PER FAR 36.602- Professional qualifications necessary for satisfactory performance of required service and working together as a team (in-house and/or consultants(s). (The firm and A/E on staff representing the project or signing drawings in each discipline must be licensed to practice in accordance with the Georgia State law requirements. Provide Professional License numbers and or proof of Licensure.) Specialized experience and technical competence in the type of work required, including, where appropriate, experience in energy conservation, pollution prevention, waste reduction, and the use of recovered materials. (Include Government and private Experience on similar projects) Capacity to accomplish work in the required time. (Provide firms experience with similar size projects and available capacity of key disciplines). Past performance on contracts with Government agencies and private industry in terms of cost control, quality of work, and compliance with performance schedules. (Government Past Performance Information Systems may be accessed) PER VAAR 836.602-1 Reputation and standing of the firm and its principal officials with respect to professional performance, general management, and cooperativeness. (Awards, Outstanding Merits, Recommendations) Record of significant claims against the firm because of improper or incomplete architectural and engineering services. Specific experience and qualifications of personnel proposed for assignment to the project and their record of working together as a team. The selected A/E must have previous experience in the design of high cost high tech medical equipment site prep within Medical Facilities. The A/E must provide documentation of at least three designs of new builds or renovations within Medical Facilities of equal scope and magnitude. The design experience submitted under this criterion must have been completed within the past three (3) years and must contain references with names, and phone numbers, and submitted along with the SF330. PAST PERFORMANCE QUESTIONNAIRE SUBJECT: Past Performance Questionnaire for Solicitation 36C24919R0112 A/E - Construct MRI & SPEC-CT HCHT Site Prep Building at the Dublin VAMC. PAST PERFORMANCE INSTRUCTIONS The Network Contracting Office (NCO) 7, Augusta, GA has issued a solicitation for the for the aforementioned project. Past performance information will be used to evaluate proposals received. Section A is to be completed by the Offeror. Section A of the enclosed questionnaire lists the contractor who has identified your organization as a source to evaluate their past performance. Section A also authorizes release of this information to NCO 7 Network Contracting Office, Augusta, GA. The Offeror must provide this entire document to each of its assessors. The Offeror shall only submit with its proposal (by the closing date of the Solicitation) copies of Section A of the questionnaire as provided to the assessors. Section B in its entirety is to be completed by the assessor(s). An individual assessor knowledgeable of the contractor s quality of supplies and services rendered is requested to verify, complete the questionnaire, and submit to the Contracting Office. If evaluating more than one contract for the same contractor, use a separate questionnaire for each contract being evaluated. Because this information is critical to the evaluation process, your time and effort in providing your assessment is greatly appreciated. The questionnaire should be completed as soon as possible but not later than the solicitation due date of July 15, 2019. Assessor is requested to send electronically to jason.kinchen@va.gov. Unacceptable (U) Based on the offeror s performance record, the Government has no reasonable expectation that the offeror will be able to successfully perform the required effort. The questions on the survey (see below) shall be rated in accordance with the definitions provided in the Rating Scale.
