- The purpose of this modification to the Pre-Solicitation is to include Statement of Work language, clarify Construction Range, add Past Performance Questionnaire, and clarify the date for 330s submission. The Working Drawings and Specifications for this design shall include, but not be limited to, areas of alterations as described in this Scope of Work. Provide all labor, materials, tools, equipment, and design services necessary to achieve the Carl Vinson VA Medical Center project, Construct Community Living Center Neighborhood to Right-Size and Support Patient Privacy. 1. This project will engage an Architectural/Engineering firm to provide Schematic Design, Design Development, Contract Drawings, Specifications, Construction Cost Estimates, and Construction Period Services. 2. Services will provide for the construction of a new Community Living Center (CLC) Neighborhood space in accordance with the Small House Model design guide to accommodate projected workload demand; while also addressing projected space gaps, supporting patient privacy, and creating a home-like environment for Veterans needing extended care at the Carl A. Vinson VAMC, Dublin, GA. The project location will be on the west side of the campus adjacent to existing CLC Neighborhoods located within Buildings 8,10 and 12; and will create between 28-34 beds. The total amount of new space constructed will be 29,000 of which 27,000 will be devoted to a Community Living Center and 2,000 SF will constitute common space. The project will also add 100 additional surface parking spaces. The estimated Construction price is over $10,000.000.00 The date and time for receipt of electronic 330s is November 7, 2019 at 2:30 PM EST. PAST PERFORMANCE QUESTIONNAIRE SUBJECT: Past Performance Questionnaire for Solicitation 36C24920R0005 A/E - Construct CLC Neighborhood to Right-Size and Support Patient Privacy at the Dublin VAMC. PAST PERFORMANCE INSTRUCTIONS The Network Contracting Office (NCO) 7, Augusta, GA has issued a solicitation for the for the aforementioned project. Past performance information will be used to evaluate proposals received. Section A is to be completed by the Offeror. Section A of the enclosed questionnaire lists the contractor who has identified your organization as a source to evaluate their past performance. Section A also authorizes release of this information to NCO 7 Network Contracting Office, Augusta, GA. The Offeror must provide this entire document to each of its assessors. The Offeror shall only submit with its proposal (by the closing date of the Solicitation) copies of Section A of the questionnaire as provided to the assessors. Section B in its entirety is to be completed by the assessor(s). An individual assessor knowledgeable of the contractor s quality of supplies and services rendered is requested to verify, complete the questionnaire, and submit to the Contracting Office. If evaluating more than one contract for the same contractor, use a separate questionnaire for each contract being evaluated. Because this information is critical to the evaluation process, your time and effort in providing your assessment is greatly appreciated.
