Page 3 of 3 SERVICESTELEVISION CABLE SIGNAL SUBSCRIPTION SERVICE This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes only the solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation#, 36C24719Q1086, is being issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2019-02 (06/05/2019). This requirement is a Full and Open Competition with 52.219-6 Notice of Total Small Business set-aside (JAN 2019. The NAICS code for this requirement is 515210 Cable and other subscription programming ($38.5 million Size Standard). Description of requirements for the items to be acquired: See attached Statement of Work (SOW). (See Atch 3) The following line items pertain to the description as stated: Price/Cost Schedule (See Atch 2). Performance Period: October 1, 2019 through September 30, 2020 (base plus 4 one-year options). All services are to be delivered to the Carl Vinson VA Medical Center, 1826 Veteran Blvd, Dublin, GA 31021. As a part of delivery and acceptance the FOB point is destination. The provision at 52.212-1, Instructions to Offerors Commercial (Oct 2015), (see Atch 1), applies to this acquisition and offers must fill out price/cost schedule attached. Offerors must submit their technical and management approach that pertains to the SOW. Offerors are to e mail complete quotes to gail.bargaineer2@va.gov no later than: 4:00 PM EST, October 28, 2019. No responses will be accepted after the response deadline. Direct all questions to gail.bargaineer2@va.gov no later than October 28, 0900 AM EST. No questions will be accepted after the deadline. All provisions and clauses applicable to this solicitation can be viewed in full text using the following web addresses: http://www.acquisition.gov/far/index.html and http://www.va.gov/oal/library/vaar/ (See Atch 3) The provision at 52.212-2, Evaluation -- Commercial Items(OCT 2014), will not be used. The comparative evaluation procedure will be used in accordance with FAR 13.106-2(b)(3). Award will be made to the lowest price technically acceptable offer. Technical acceptability will be based upon offerors submitting sufficient evidence of adequate equipment, personnel and necessary experience to establish responsibility and integrity to meet the requirements of this contract. Any offeror, who does not have adequate experience, personnel and/or equipment, in the opinion of the Contracting Officer, will be rejected. To assist in this determination, offeror is required to furnish the following information: To be eligible for award offerors must be registered in System for Award Management (SAM) in accordance with the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items. Past Performance - Offerors will be evaluated on their ability to perform the contract successfully. Past performance evaluation will consider reliability of past performance information, source of information, and the relevance of information. Offerors without relevant past performance or for whom past performance information is not available, will receive a neutral rating. Relevant past performance information shall include key personnel who have relevant experience, predecessor companies, and subcontractors who will perform major elements of this solicitation. Provide documentary evidence that your company s primary business is Television Cable Signal Subscription Service and that your company has actually performed and provided Television Cable Signal Subscription Service for the past three years. Failure to provide documentation that your company can provide Television Cable Signal Subscription Service as described above will render your proposal nonresponsive. Provide at least three references whereby you have provided the same or similar services, of the magnitude described in this solicitation, to include company name, point of contact, current telephone number, type of contract, and period of performance. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, apply to this acquisition and no additional addenda to the clause included. (End of Clause) The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items, applies to this acquisition and no additional FAR clauses cited in the clause are applicable to the acquisition. (End of Clause) The clause at 52.217-8, Option to Extend Services-- The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 60 days. (End of Clause) The clause at 52.217-9, Option to Extend the Term of the Contract--(a) The Government may extend the term of this contract by written notice to the Contractor within 60 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years.
