Attachment 3 - Past Performance Questionnaire Attachment 5 - Work Order Price List Attachment 4 - Financial Institution Reference Sheet Attachment 1 - Grounds Maintenance Performance Work Statement Attachment 2 - Wage Determination 2015-5419 Solicitation, FA2543-20-R-0002 This is a combined synopsis/solicitation for commercial service prepared in accordance with FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written Solicitation (SF-1449) Number FA2543-20-R-0002, Request for Proposal (RFP) is issued as an attachment to this notice. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-01, effective 10 October 2019 and the Defense Federal Acquisition Regulations (DFARS) change notice 20191001. This RFP is issued as a Competitive HUBZone Set-Aside requirement; only those firms certified by the U.S. Small Business Administration as HUBZone firms are eligible for award. The North American Industrial Classification System (NAICS) Code is 561730 with a small business size standard of $8 million. The contractor shall provide non-personal services to include all personnel, equipment, tools, materials, supervision and other items and services necessary to provide grounds maintenance services at Buckley Air Force Base (AFB), Colorado in accordance with the Performance Work Statement (PWS), which is also an attachment to this notice and shall become a part of the resulting contract. The contractor shall comply with applicable federal, regional, state, and local laws and commercial standards, and must possess all required licenses to perform the services. Work will include, but not is limited to the following functional areas: (1) Maintain Improved Grounds; (2) Maintain Semi-Improved Grounds; (3) Maintain Un-Improved Grounds; (4) Maintain Vegetative Beds, Inert Beds and Related Areas; (5) Maintain Surface Drainage Ditches; and (6) Other Special Requirements (to include tree pruning/trimming, xeriscaping and urban forest services, special cuts, planting trees, shrubs, and vegetative beds, removal of shrubs and vegetative beds, and performing emergency and special event services). This requirement is subject to the Service Contract Labor Standards and will include the appropriate Department of Labor Wage Determinations. This requirement is a Performance-Based, Firm Fixed-Price (FFP) Contract; the period of performance will be one (1) base year, with four (4) sequential 12-month options, for a total contract period of sixty (60) months. If necessary, another 6-month option may be exercised in accordance with FAR 52.217-8 Option to Extend Services. Award will be granted to the responsible offeror whose proposal conforms to the solicitation and who is considered to be the most advantageous to the Government according to FAR Part 12, Acquisition of Commercial Items, and FAR Part 13 Simplified Acquisition Procedures, utilizing the Lowest-Price Technically-Acceptable (LPTA) source selection process. The following factors shall be used to evaluate the proposals: (1) Technical Acceptability, and (2) Price, and (3) Past Performance. Technical acceptability shall be evaluated on a technically acceptable or not technically acceptable basis. Price evaluation shall be based on the lowest reasonable, balanced, and overall total evaluated price (TEP). Past performance will be evaluated as acceptable or unacceptable. Evaluation of proposal may be made without discussions with the offeror(s). Additional and specific instructions in preparing the proposal and evaluation basis for award are provided in the attached written solicitation package (SF-1449). To be eligible for a contract award on this requirement, the contractor must be registered with the System for Award Management (SAM) at www.sam.gov. It is the responsibility of each potential offeror to periodically check www.fbo.gov to obtain any related amendments to this solicitation. It is also the offerors' responsibility to be familiar with the applicable clauses and provisions that apply to this acquisition. These clauses and provisions and any applicable addenda are stipulated in the solicitation document (SF-1449). A site visit is scheduled at 1:00pm to 3:00pm Mountain on Thursday, 7 November 2019. Interested parties wanting to attend this site visit shall make an arrangements with Ms. Jenny L. Meadlin, Contract Specialist, via email jenny.meadlin@us.af.mil, telephone (720) 847-6674 no later than 2:00PM MDT on Thursday, 31 October 2019. Requests shall include the Company Name, Full Name, Driver's License Number and State, and Date of Birth. No more requests for site visits may be accepted after this deadline time/date. The site visit will be held on 7 November 2019 at 1:00 PM Mountain at Buckley AFB. The meeting location will be: Leadership Development Center 600 South Aspen Street Buckley AFB, CO 80011 Request for Information (RFI). Requests for information must be made in writing, and submitted to Ms. Jenny L. Meadlin, Contract Specialist, by email after the scheduled site-visit, but no later than 3:00PM MDT on Monday, 25 November 2019. No RFIs will be accepted after this deadline time/date. Deadline for Submission of Proposals. Proposals must be submitted to Ms. Jenny L. Meadlin, Contract Specialist, at the previously provided telephone and email address, no later than the deadline receipt of proposals as established in block 8 of the solicitation. Faxed, e-mail, or printed offers will not be accepted. Contractors may submit offers electronically to the Government via the DoD SAFE site (https://safe.apps.mil/). All documents submitted must be saved in.pdf format, and shall follow the instructions in FAR 52.212-1. Title of the documents should include the offerors CAGE code (i.e., "Part 1 Tech Proposal CAGE#"). Offers may be hand carried submissions of compact disks (CDs) containing electronic files. Contractors should request base access for hand carried offer delivery NLT 24 hours prior to the offer due date/time by sending an email request identifying the company name and list of individuals requesting access to the Contracting Officer identified below. Hand carried or common carrier delivered proposal must arrive at the Buckley AFB Contracting Office no later than the deadline for receipt of proposals as established in block 8 of the solicitation. The package used to submit your proposal must show the time and date specified for receipt, the Solicitation Number, and the name and address of the offeror.
