Description
PERFORMANCE WORK STATEMENT This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. All vendors must be registered in the System for Award Management (SAM). The solicitation number is W912JA-20-R-0006 is issued as a Request for Proposal (RFP) unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2019-06. The associated North American Industrial Classification System (NAICS) code for this procurement is 561320 Temporary Help Services with a small business size standard of $30.0 million dollars.. This requirement is a Service Disabled Veteran Owned Small Business set-aside and only qualified offerors may submit bids. Requirement: Provide Role Players, Moulage and Weighted Manikins, Ambulance, Rubble Pile reset, Lumber Package and Porta Let/handwashing Station Services in support of The Alabama National Guard CERFP Chemical Training Exercise. Performance Work Statement (PWS) is attached. Period/ Place of Performance: 9 -12 December 2019 at Fort McClellan, Anniston, AL (Pelham Range) Discount Terms: NET 30 Offers are due by 15 November 2019, @1500 CST at amelia.p.davis.civ@mail.mil. Submit all questions NLT 8 November 2019 @1500 CST. The following provisions below applies to this acquisition: 52.212-1, INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS (OCT 2018) (a) North American Industry Classification System (NAICS) code and small business size standard. However, the small business size standard for a concern which submits an offer in its own name, but which proposes to furnish an item which it did not itself manufacture, is 500 employees. (b) Submission of offers. Submit signed and dated offers to the office specified in this solicitation at or before the exact time specified in this solicitation. Offers may be submitted on a letterhead stationery, or as otherwise specified in the solicitation to amelia.p.davis.civ@mail.mil. As a minimum, offers must show-- (1) The solicitation number; (2) The time specified in the solicitation for receipt of offers; (3) The name, address, and telephone number of the offeror; (4) NAICS code; (5) Price and any discount terms; (6) "Remit to" address, if different than mailing address; (7) A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically); (8) Acknowledgment of Solicitation Amendments; (9) Past performance information, when included as an evaluation factor, to include recent and relevant contracts for the same or similar items and other references (including contract numbers, points of contact with telephone numbers and other relevant information); and (10) If the offer is not submitted on the SF 1449, include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration. (c) Period for acceptance of offers. The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation. (d) Product samples. When required by the solicitation, product samples shall be submitted at or prior to the time specified for receipt of offers. Unless otherwise specified in this solicitation, these samples shall be submitted at no expense to the Government, and returned at the sender's request and expense, unless they are destroyed during pre-award testing. (e) Multiple offers. Offerors are encouraged to submit multiple offers presenting alternative terms and conditions, including alternative line items (provided that the alternative line items are consistent with subpart 4.10 of the Federal Acquisition Regulation), or alternative commercial items for satisfying the requirements of this solicitation. Each offer submitted will be evaluated separately. (f) Late submissions, modifications, revisions, and withdrawals of offers: (1) Offerors are responsible for submitting offers, and any modifications, revisions, or withdrawals, so as to reach the Government office designated in the solicitation by the time specified in the solicitation. If no time is specified in the solicitation, the time for receipt is 4:30 p.m., local time, for the designated Government office on the date that offers or revisions are due. (2)(i) Any offer, modification, revision, or withdrawal of an offer received at the Government office designated in the solicitation after the exact time specified for receipt of offers is "late" and will not be considered unless it is received before award is made, the Contracting Officer determines that accepting the late offer would not unduly delay the acquisition; and-- (A) If it was transmitted through an electronic commerce method authorized by the solicitation, it was received at the initial point of entry to the Government infrastructure not later than 5:00 p.m. one working day prior to the date specified for receipt of offers; or (B) There is acceptable evidence to establish that it was received at the Government installation designated for receipt of offers and was under the Government's control prior to the time set for receipt of offers; or (C) If this solicitation is a request for proposals, it was the only proposal received. (ii) However, a late modification of an otherwise successful offer, that makes its terms more favorable to the Government, will be considered at any time it is received and may be accepted. (3) Acceptable evidence to establish the time of receipt at the Government installation includes the time/date stamp of that installation on the offer wrapper, other documentary evidence of receipt maintained by the installation, or oral testimony or statements of Government personnel. (4) If an emergency or unanticipated event interrupts normal Government processes so that offers cannot be received at the Government office designated for receipt of offers by the exact time specified in the solicitation, and urgent Government requirements preclude amendment of the solicitation or other notice of an extension of the closing date, the time specified for receipt of offers will be deemed to be extended to the same time of day specified in the solicitation on the first work day on which normal Government processes resume. (5) Offers may be withdrawn by written notice received at any time before the exact time set for receipt of offers. Oral offers in response to oral solicitations may be withdrawn orally. If the solicitation authorizes facsimile offers, offers may be withdrawn via facsimile received at any time before the exact time set for receipt of offers, subject to the conditions specified in the solicitation concerning facsimile offers. An offer may be withdrawn in person by an offeror or its authorized representative if, before the exact time set for receipt of offers, the identity of the person requesting withdrawal is established and the person signs a receipt for the offer. (g) Contract award (not applicable to Invitation for Bids). The Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror's initial offer should contain the offeror's best terms. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The Government may reject any or all offers if such action is in the public interest; accept other than the lowest offer; and waive informalities and minor irregularities in offers received. (h) Multiple awards. The Government may accept any item or group of items of an offer, unless the offeror qualifies the offer by specific limitations. Unless otherwise provided in the Schedule, offers may not be submitted for quantities less than those specified. The Government reserves the right to make an award on any item for a quantity less than the quantity offered, at the unit prices offered, unless the offeror specifies otherwise in the offer. (i) Availability of requirements documents cited in the solicitation. (4) Nongovernment (voluntary) standards must be obtained from the organization responsible for their preparation, publication, or maintenance. (j) Unique entity identifier. (Applies to all offers exceeding $3,500, and offers of $3,500 or less if the solicitation requires the Contractor to be registered in the System for Award Management (SAM).) The Offeror shall enter, in the block with its name and address on the cover page of its offer, the annotation ``Unique Entity Identifier'' followed by the unique entity identifier that identifies the Offeror's name and address. The Offeror also shall enter its Electronic Funds Transfer (EFT) indicator, if applicable. The EFT indicator is a four-character suffix to the unique entity identifier. The suffix is assigned at the discretion of the Offeror to establish additional SAM records for identifying alternative EFT accounts (see subpart 32.11) for the same entity. If the Offeror does not have a unique entity identifier, it should contact the entity designated at www.sam.gov for unique entity identifier establishment directly to obtain one. The Offeror should indicate that it is an offeror for a Government contract when contacting the entity designated at www.sam.gov for establishing the unique entity identifier. (k) Reserved. (l) Debriefing. If a post-award debriefing is given to requesting offerors, the Government shall disclose the following information, if applicable: (1) The agency's evaluation of the significant weak or deficient factors in the debriefed offeror's offer. (2) The overall evaluated cost or price and technical rating of the successful and the debriefed offeror and past performance information on the debriefed offeror. (3) The overall ranking of all offerors, when any ranking was developed by the agency during source selection. (4) A summary of the rationale for award; (5) For acquisitions of commercial items, the make and model of the item to be delivered by the successful offeror. (6) Reasonable responses to relevant questions posed by the debriefed offeror as to whether source-selection procedures set forth in the solicitation, applicable regulations, and other applicable authorities were followed by the agency. (End of provision) ADDITIONAL INSTRUCTIONS TO OFFERORS: 1. Proposals are to consist of three separate volumes (3 electronic files), volume 1, volume 2, and volume 3. a. Do not provide physical binders. We require 3 electronic files as described below. b. Offerors are to furnish proposals via email to Ms. Philanthia Relf at philanthia.c.relf.mil@mail.mil and amelia.p.davis.civ@mail.mil. Request received and/or read receipts as desired to confirm delivery. c. All narrative and spreadsheet writing is to be written using Font New Times Roman 12 point font. 2. Volume 1 contains technical and past performance data. a. Technical. i. The volume 1 file is to be titled "Volume 1". ii. The total Technical page count limit is 10 pages. If this volume exceeds the 10-page count limit, evaluators will not evaluate beyond page 10. And, the proposal may be found non-responsive/unawardable as a result. iii. The Technical portion of Volume 1, at a minimum, must address the following Technical Subfactors: 1. Subject Matter Experts (SME). While resumes are not required for this, the proposal must demonstrate the relevant experience or expertise possessed by the SME's an Offeror plans to utilize in support of this event. 2. The Offeror's plan on meeting the role player, weighted manikin, and moulage requirements of the PWS. 3. Narrative on how the Offeror will satisfy security requirements and safety measures described in the PWS. b. Past Performance. i. The Past Performance portion of Volume 1 does not count against the 10 page limit imposed on the Technical portion. ii. Total Past Performance page count limited to 2 pages. Past Performance information exceeding the 2 page limit will not be evaluated. iii. Write a one paragraph summary for each past performance action you wish to include in your proposal. Address the scope of the work, dollar value (magnitude), and the time frame of the action. 3. Volume 2 is for pricing data only. a. Volume 2 is to be in Excel format, not PDF. It is to be titled "Volume 2". b. In addition to providing the Offeror's overall price for the event, the proposal must provide breakout pricing for the following items: i. NIMS/ICS Subject Matter Experts ii. Role Players, Moulage, and Trauma Manikins iii. Radiation Buttons iv. Rubble Pile Reset v. Chemical Stimulants vi. Ambulances, Paramedic/EMT personnel c. Format: i. Use row 1 to identify the offeror's name. ii. Use column A for the line items. iii. Use column B for the price. iv. Format column B as currency. d. Proposals which do not follow this pricing format may be found non-responsive. 4. Volume 3 is a single statement file containing a signed document that acknowledges the terms and conditions of the solicitation as well as any amendments that have been implemented. These signed pages are to be furnished as a single electronic file titled "Volume 3". END OF "ADDITIONAL INSTRUCTIONS TO OFFERORS" 52.212-2, EVALUATION--COMMERCIAL ITEMS (OCT 2014) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i) Technical capability of the service offered to meet the Government requirement; (ii) past performance (iii) price. Technical and past performance, when combined, are approximately equal to price. 1. Technical. The Technical factor will be evaluated and assigned an overall rating based on these subfactors: a. Qualifications of SME's (Subject Matter Experts). Proposals which exhibit SMEs an Offeror is bringing to this event who possess real world experience with first responder backgrounds will be rated higher than proposals which do not. b. The Offeror's plan on how it will satisfy the requirements for role players, weighted manikins, and moulage artist support for this event. c. How the Offeror will satisfy the security and safety requirements measures described in the PWS. The Technical factor will be assigned an overall rating using one of the following ratings: Adjectival Rating Description - Outstanding Proposal indicates an exceptional approach and understanding of the requirements and contains multiple strengths, and risk of unsuccessful performance is low. - Good Proposal indicates a thorough approach and understanding of the requirements and contains at least one strength, and risk of unsuccessful performance is low to moderate. - Acceptable Proposal meets requirements and indicates an adequate approach and understanding of the requirements, and risk of unsuccessful performance is no worse than moderate. - Marginal Proposal has not demonstrated an adequate approach and understanding of the requirements, and/or risk of non-performance is high. Unacceptable Proposal does not meet requirements of the solicitation, and thus, contains one or more deficiencies, and/or risk of unsuccessful performance is unacceptable. Proposal is un-awardable. 2. Past Performance. This consists of two parts. Offers are to provide a summary of past work within these areas: - Relevance - Relevance addresses how closely past work associates with the current requirement. - Recency - Performance older than 3 years may not be rated as highly as performance in the past year. There is no minimum number of projects or contracts to be listed. Provide narrative regarding the size of the requirements (scope and dollar magnitude) and to the kind of customer (military, etc...). References are encouraged. Relevance will be judged on the following scale: Past Performance Relevance Ratings Rating Definition - Very Relevant Present/past performance effort involved essentially the same scope and magnitude of effort and complexities this solicitation requires. - Relevant Present/past performance effort involved similar scope and magnitude of effort and complexities this solicitation requires. Somewhat - Relevant Present/past performance effort involved some of the scope and magnitude of effort and complexities this solicitation requires. Not Relevant Present/past performance effort involved little or none of the scope and magnitude of effort and complexities this solicitation requires. Based on evaluation results of the 2 Past Performance Subfactors, the Past Performance Factor will be assigned one of the following Performance Confidence Assessments: Performance Confidence Assessments Rating Definition - Substantial Confidence Based on the offeror's recent/relevant performance record, the Government has a high expectation that the offeror will successfully perform the required effort. - Satisfactory Confidence Based on the offeror's recent/relevant performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort. - Neutral No recent/relevant performance record is available or the offeror's performance record is so sparse that no meaningful confidence assessment rating can be assigned. The offeror may not be evaluated favorably or unfavorably on the factor of past performance. - Limited Confidence Based on the offeror's recent/relevant performance record, the Government has a low expectation that the offeror will be able to successfully perform the required effort. - No Confidence Based on the offeror's recent/relevant performance record, the Government has no expectation that the offeror will be able to successfully perform the required effort. 3. Price. While price is an important factor, it is not more important than the other factors. Price must be reasonable and it must reflect market pricing. Pricing that is too low may be deemed as unrealistic, and could be interpreted to mean the offeror does not account for all of the cost implications of the work. The Government intends to award without discussion. (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
