Market research was conducted and determined that Olympus America, Inc is the only vendor capable of providing the Pulmonary Scopes service necessary that meet the requirements. All items are proprietary to Olympus America, Inc and not able to be sourced out to another. This Notice of Intent is not a request for Proposals or Quotations. No solicitation document exists and no telephone or email inquiries will be accepted. No contract award will be made on the basis of proposals, or quotations received in response to this notice. The government MAY consider all Expressions of Interest (EOI) received prior to 12 November 2019. Please include your capability statement. Capabilities Statement should include: 1. Company name, address, point of contact, phone number, Dunn & Bradstreet number, and e-mail address; 2. Evidence of SDVOSB or VOSB status through registration at VIP (https://www.vip.vetbiz.gov/), if appropriate, 3. Evidence of ability to offer under NAICS Code 711510 and listing in the System for Award Management (www.SAM.gov), including a copy of the representations and certifications made in that system, 4. A summary describing at least three (3) projects of similar size and scope completed in the past five (5) years that demonstrate your company s experience to satisfy the requirements of the SOW (3-page limit), 5. Photos of work related to the requirements of the SOW as appropriate. A determination by the government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement. Responses to this notice must be emailed to carlton.lunsford@va.gov and received by 10:00AM EST on 12 November 2019. Submitters are responsible for ensuring their EOI reaches the designated contracting officer on-time. STATEMENT OF WORK A. Background: The James J. Peters VA Medical Center, Pulmonary Service, is requesting to lease pulmonary scopes to perform pulmonary procedures, and to obtain a full-service agreement that will cover all necessary corrective maintenance for all the newly leased pulmonary scopes. The service agreement will begin when all scopes are accepted on site. B. Objectives: The objective is to lease state-of-the-art scopes that will be used as outlined above. The service agreement must make available loaners, if available, and cover all associated costs of any repairs (including shipping, parts, and labor) for the equipment outlined above through a full-service agreement, to ensure safe and reliable performance of pulmonary scopes at James J. Peters VAMC. Lease term pricing shall be based on a 36-month lease. C. Period of Performance: Lease and maintenance service shall be for a period of twelve months with two option years. D. Place of Performance: James J. Peters VA Medical Center, 130 W. Kingsbridge Road, Bronx, NY 10468. The scopes will be used for procedures with the service and shall be delivered to Sterilization Services. E. Product Specification: The delivery order will include the following endoscopes in the quantities specified: 1) BF-UC180F-A : BF-UC180F-A ULTRASOUND BRONCHOSCOPE X1 2) BF-MP190F : ULTRA-SLIM BRONCHOSCOPE X1 F. Government-Furnished Property, Material, Equipment, or Information: None G. Security: No special security requirements H. Safety Codes / Certification / Licensing: No special requirements I. Special Material Requirements: None J. Other Requirements: This delivery order includes an option to buy all scopes at the end of the lease agreement as specified in the purchase option.
