- Performance Work Statement (PWS) for the Employee Assistance Program (EAP) Service THIS IS A PRE-SOLICITATION NOTICE ONLY. A SOLICITATION IS NOT CURRENTLY AVAILABLE This requirement is being issued as a 100% Small Business Set-aside. (Ref: FAR 19.5) This requirement will be competed and awarded under Federal Acquisition Regulation (FAR) Part 13, Simplified Acquisition Procedures. The Government intends to select one Contractor from the subject solicitation. Description of the Service Requested: The contractor shall provide all personnel, equipment, tools, materials, supervision, and quality control necessary to perform a wide range of short-term diagnostic assessment, intervention, counseling and referral services for various issues affecting employee mental and emotional well-being, which may adversely impact the employee's job performance not limited to work-related difficulties such as alcohol and other substance abuse, stress, grief, depression, anxiety, marital and family problems, eating disorders, financial/credit problems and legal concerns, as defined in this PWS. Long-term treatment and rehabilitation is not within the scope of this contract. The North American Industry Classification System (NAICS) code for this service is 624190. The small business size standard is $11 million average annual revenue for the previous three years. The period of performance is for one year starting on/or about the 1 April 2020.The resulting contract will be a firm-fixed price with a base year period and four one-year option periods. By submitting a quotation in response to the RFQ, contractors validate their quotation will be valid for 90 days from the RFQ due date. The USP&FO for Maine intends to issue a Request for Quote (RFQ) under W912JD-20-Q-0001 on/or about 25 November 2019. The information will be made available to the public solely distributed through the General Services Administration's Federal Business Opportunities website at www.fedbizopps.gov/. Hard copies of the solicitation will not be available. This site provides downloading instructions. All future information regarding this acquisition, including solicitation amendments, will also be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about the acquisition. All inquiries must be in writing, preferred method is via email to the person(s) specified in the solicitation. All answers will be provided in writing via posting to the web. This announcement serves as the advance notice for this project. All interested parties submitting proposals that do not have an active account in System for Award Management database at http://www.sam.gov website will not be eligible for award.
