- The VA Western New York Healthcare System (VAWNYHS) within the New York/New Jersey VA Health Care Network (VISN 2), has a need for capitated Community Based Outpatient Clinic (CBOC) services providing primary care and certain specialty services in a private hospital, office, or clinic environment for the Veterans of Erie County, New York. Contractor is required to provide two facilities a main CBOC located within the city limits of Lackawanna, NY, or within the town limits of Southern West Seneca, NY (Route 16 Center Road or further South), as well as a satellite outreach clinic facility which must be located within the village limits of Springville, NY, which is located in Southern Erie County, NY. Veterans to be serviced under this contract reside primarily in Erie County, however; residency is not restricted to this county. It is estimated that Request for Proposal (RFP) 36C24219R0014 will be available for download from the Contract Opportunities website at https://beta.sam.gov/ on or about November 22, 2019. No hardcopies will be sent. Questions must be submitted in writing via email to the Contract Specialist at Lauren.Helming@va.gov. Questions must be received a minimum of ten (10) business days before the closing date listed in the RFP. If warranted, responses to inquiries will be incorporated into a written amendment posted to the Contract Opportunities website at https://beta.sam.gov/. Please note, Contract Opportunities is where the Federal Business Opportunities website (http://www.fbo.gov) will be moved starting on November 8, 2019. For information regarding the transition to Contract Opportunities, please review the information available on https://www.fbo.gov/. Verbal questions and phone calls will not be addressed. Offerors are encouraged and are responsible for routinely checking the Contract Opportunities website for any solicitation amendments that may be issued throughout the proposal period. This requirement is being solicited as unrestricted. However, it is noted that the solicitation will contain VAAR 852.215-70, Service-Disabled Veteran-Owned and Veteran-Owned Small Business Evaluation Factors (JUL 2016) (DEVIATION). The NAICS Code for this requirement is 621498 All Other Outpatient Care Centers and the Size Standard is $20.5 Million. Contractors are required to be registered with the System for Award Management (SAM) at https://www.sam.gov/portal/public/SAM prior to contract award.
