- Page 3 of 3THIS REQUEST FOR INFORMATION IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION.This is a Request for Information (RFI), this is not a Request for Quotes (RFQ) or Request for Proposals (RFP). No formal solicitation document exists at this present time. This is not a solicitation announcement. The purpose of this Sources Sought notice is for market research to gain knowledge of potential vendors qualified under NAICS code search for DC Hematology Cost per test of 334516, includes Lab equipment Manufacturing laboratory equipment with a NAICS code size standard of 1000 employees. The requirement period of performance may be planned for a base year only.Responses to this Sources Sought notice will be utilized by the Government to make appropriate acquisition decisions. After review of the responses to this Sources Sought notice, a solicitation announcement may or may not be published in FedBizOpps (FBO).REQUESTED INFORMATIONIf you are interested, and can provide the required supplies/services, please provide the requested information as indicated below. Responses to this Sources Sought notice shall also include: company name, address, point of contact, size of business:- Does your company have an FSS contract with GSA or the NAC or are you a contract holder with NASA SEWP or any other federal contract? If so, please provide the contract type and number.- Please indicate the size status and representations of your business, such as but not limited to: Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) with an active registration in VETBIZ, HUBZone, Woman Owned Small Business (WOSB), Small Business, Large Business, etc.).LINE ITEM:1 The Contractor shall provide to 1 YR the laboratory full instrumentation, installation, correlation studies, reagents, maintenance and preventive services and a quality control program to provide routine and stat Hematology assays on a cost per reportable basis.SOWDAYS AND HOURS OF OPERATION: Monday - Sunday, 24 hours a day including federal holidays.EQUIPMENT DESCRIPTION: Three Hematology analyzers, UPS s, printers, monitors and any other equipment required to produce complete blood counts with automated differentials, reticulocyte counts and an early sepsis indicator. PERFORMANCE REQUIREMENTS: The Contractor shall provide all instruments, test equipment, reagents, quality control program, training programs for personnel, repair/replacement service and parts, necessary to perform Hematology testing in the Core Lab and the Hematology Clinic at The Pathology & Laboratory Medicine Services, VAMC DC.CONFIGURATION AND LAYOUT: The vendor must provide two identical analyzing systems and one lower volume throughput testing system that will fit in existing areas.The systems must conform to NCCLS/ industry standards.The vendor must have a Business Associate Agreement and an Interconnection Security Agreement with the VA.SOFTWARE:The system shall have the capacity to connect to the current VA LIS system (VISTA). Interface software will be provided.Bi-directional LIS interface capabilitySystem provides onboard reagent tracking to monitor lot numbers, expiration dates and volumes.A comprehensive on board QC program, including troubleshooting or system validation of QC material. The system must have quality control data storage and reporting capabilities, the ability to view and print daily and monthly QC results, view and print Levy Jennings graphs, visually alert the operator of QC failures and to document out of range QCAutoverification rules package through a middleware software or through the existing middleware, Data Innovations, which already is in place at the VA. SPECIFICATIONS FOR CELLUAR ANALYSIS SYSTEM:The system aspirates whole blood or body fluid samples from specimen tubes or vials.The system must display a high sensitivity and specificity.A single aspiration pathway is used for automated and manual sample introduction.Digital bar code technology is used to read labeled sample tubes.The system must be able to accommodate multiple sized tubes in a single cassette.All reagents must be able to be changed while the instrument is running.System can run a complete CBC with automated differential, NRBC and reticulocytes on a very small sample (less than 200 uL).System has on-board algorithms that use flagging criteria to minimize bias.System must use flow cytometric digital morphology to optimize performanceCBC must include WBC, RBC, HGB, HCT, MCV, MCH, MCHC, RDW, PLT, and MPVAutomated differential must include absolute and percentage counts for Neut, Lymph, Mono, Eos, Baso, and NRBC.Reticulocyte count includes absolute and percentage, as well as MRV and IRF.Directly measure MCV s and MPV s.Early Sepsis Indicator Capable of identifying the lineage of blastsAll required tests must be FDA approved for patient testingSYSTEM MAINTENANCE:User performed system maintenance is minimal, taking less than 5 minutes per day and less than 15 minutes per week.SERVICE: All service cost to be incurred by vendorThe contractor will provide on-site service 24 hours a day and 7 days a week, year round to include weekends and holidays.The Contractor shall respond to all emergency service calls within 2 hours.The Contractor shall provide a toll-free number for service calls and technical assistance 24 hours a day.The Contractor shall make all repairs as expeditiously as possible, so as not to compromise patient care.The Contractor shall run quality control material after all repairs and wait until all results are within range to verify a successful repair.The contractor shall provide new instruments and equipment, actively marketed and on the GSA contract.The contractor shall provide preventive maintenance inspection, testing and calibration of equipment and instruments as deemed necessary by the Laboratory and to ensure all equipment furnished is maintained in excellent working condition.All instruments will be supported by a UPS power supply to maintain power in the event of a power outage.The contractor shall maintain service repairs and provide a full report of any repairs to the Contracting Officer's Representative (COR).Ownership of analyzers will remain with the contractor. Analyzer's rental charges are absorbed through cost per test charges.Contractor shall provide support personnel to correlate existing assays to new methodology and switch to new equipment.The contractor shall provide off-site initial training for one key operator per cellular analysis instrument. Costs associated with training including room, board and airfare are the responsibility of the contractor.The contractor shall provide off-site annual training for one operator for each completed year of cellular analysis system contract. Costs associated with training including room, board and airfare are the responsibility of the contractor.The vendor has remote diagnostics capabilities and various service and support options to meet customer needs.The contractor shall provide delivery and set up of instruments.No hardware, containing any VA patient information is to be removed from the VA, without the approval and removal of data by the VA information technology department. Failure to comply with this VA requirement may result in prosecution.CONTRACT PERFORMANCE MONITORING: The government reserves the right to monitor services and performance of instruments in accordance with the standards set forth by CLIA, the College of American Pathology and the Joint Commission. The Contracting Officer Representative (COR) is assigned to manage the contract. The COR and/or an authorized representative will be the direct point of contact for the contractor. Only the COR (or his/her authorized representative) has the authority to make requests of the vendor within the scope of the contract during the course of the contract.Unless otherwise directed by the using facility, contractor owned equipment shall be installed by the effective date of the contract.Contractor installed equipment shall remain the property of the contractor and shall be removed upon termination of the contract, when directed by the ordering facility and in full cooperation with the succeeding contractor, so as to avoid interruption of patient services.NOTESAll Capability Statements/Responses to this Sources Sought notice shall be submitted via email only to stephanie.henderson3@va.gov.
