STATEMENT OF WORK580-20-1-052-0039Beaumont Lobby and Patient Seating1.0 GENERAL SCOPEThe Veterans Health Administration (VHA) has a requirement for healthcare furniture for one of the clinics of the Michael E. Debakey VA Medical Center, in Houston, Tx. The clinic is located in Beaumont, TX.This requirement meets the VHA IDIQ specifications and high-quality threshold, while integrating excellent environmental stewardship. This requirement includes healthcare furniture, delivery, and professional installation. Service required is detailed in SV1 Healthcare Furniture technical requirements per VHA Healthcare Furniture multiple award IDIQ. Healthcare furnishings include single lounge seating, bariatric lounge seating, cylinder end tables with electrical outlets and guest seating to exam rooms and offices.2.0 PRODUCT SPECIFICATIONBrand Name Nemschoff is approved under the VHA Healthcare Furniture multiple award IDIQ contract and are listed in the IDIQ Minimal Technical Requirements (MTR) list. Nemschoff or compatible Equal, to meet the needs of furnishings seating for lobbies and end tables. Sitonit is approved under the VHA Healthcare Furniture multiple award IDIQ contract and are listed in the IDIQ Minimal Technical Requirements (MTR) list. Sitonit or compatible Equal, to meet the needs of furnishings guest seating.New purchases shall allow outpatient facilities new seating options that meet infectious control standards. 3.0 PERFORMANCE/SUBMITTALS REQUIREMENTSThe specification covers the requirement for main waiting room furniture and guest seating for exam rooms and offices of the Beaumont CBOC. The minimum acceptable requirements for manufacturer s extent of project line, design, materials, workmanship, performance, safety and services are set forth, hereinafter. Failure to meet the minimum acceptable requirements may result in disqualification of the bid. All furnishings shall be of a design, material and workmanship to withstand harddaily usage over an extended life with a minimum or maintenance and repair.The furniture dealership shall provide and be responsible for the technicalassistance, development and generation for final specifications.The manufacturer will guarantee product for a limited lifetime warranty.The contractor shall provide literature and specifications of furniture offered in bid current brand name furniture, Nemschoff Anderson chairs, cylinder tables and Sitonit Freelance arm and armless chairs or equal.Only include Nemschoff or equal, products specified within the IDIQ Contract.Only include Sitonit or equal, products specified within the IDIQ Contract.Submit price list for all product BidSubmit an installation schedule including an estimate of man- hours required for installation.Submit a furniture schedule and Bill of Materials (BOM) tagged per quantities and clinics.Submit shop drawings for all custom product, complete manufacturer's product specification and any drawings and samples required for approval prior to commencement of manufacturing.Submit all furniture warranties and maintenance manuals to owner.Coordinate and provide all products and services to manage the order, ship, deliver and install new furniture from the manufacturer until final acceptance by VA interior designer, Linda Mendeloff, ASID, NCIDQ, TRID. linda.mendeloff@va.gov. 713-791-1414 x 25468 and Juanelle B. Barnes, juanelle.bradford@va.gov, 713 791-1414 x 22834.All items shall be installed by Houston Installation Services (HIS), as they are our Certified Professional Installers. The crew members are familiar with the VA outpatient clinic; the HIS company originally installed this clinic and have proven to maneuver through hospital environments safely and with minimal disruptions. They are certified installers and allow warrantees to be intact.4.0 DELIVERY AND INSTALLATIONAll deliveries shall be to VA Beaumont Outpatient Clinic, 3420 Veterans Cir., Beaumont, Texas 77707. Outside contractor (HIS crew) shall meet furniture trucks at job site to unload/unpack, inspect and install furniture. MEDVAMC COR shall be notified by email of order ship date and projected delivery to clinic. COR shall notify outside contractor (HIS) install team of delivery date. The most current estimate of delivery need is January 21st, 2020. All furnishings must be in working condition and completely assembled. Allassembly and installation work involved in placing furniture shall be completed bythe install contractor hired by awarded contractor.Each item must be clean and free from dust, packing marks, tags, labels, etc.The furnishings shall be neat in appearance and from defects. Use no shims,wedges or similar job fashioned materials.Protection of carpet, vinyl tile, walls, and ceiling are the responsibility of theinstallers during installation. Any damage shall be replaced with like material andfinish at no cost to the Owner.Maintain areas free of waste materials, debris, and rubbish. Maintain site in aclean and orderly condition. All waste materials removal is the responsibility of the install contractor. Job site dumpsters are not available for waste disposal.Use cleaning materials which do not create hazards to health or property andwhich will not damage surfaces. Use only those cleaning materials and methodsrecommended by manufacturer of the surface material to be cleaned. Usecleaning materials only on surfaces recommended by cleaning materialmanufacturer.Provide for all dumpsters, haul fees and dump charges as required. Do not useOwners collection facilities at any time.4.1 PERIOD OF PERFORMANCE AND WORKING HOURS All work shall be completed between 8:00 a.m. and 4:30 p.m. Monday Friday. All federal holidays, excluded. The Government maintains the option to modify the normal work week, days and hours, as necessary for the accomplishment of VHA mission.5.0 INSPECTION AND ACCEPTANCEa. The order shall be inspected by the receiving company (HIS) for any damage when received at job site. b. Any missing and or damaged product shall be reported to COR; Contractor shall file the necessary paper work for follow up.c. Awarded contractor shall provide paperwork of completion dates, plus paperwork if there is a punch list, replacement parts, and/or short ship items from the manufacturer(s). d. The VA COR shall ensure all work is completed satisfactorily prior to acceptance. e. Any disputes shall be addressed with the Contracting Officer to resolve. 6.0 DELIVERY REQUIREMENTSa. Product shall be factory packed with adequate protection to all sides and corners to prevent transit and handling damage.b. Receiving contractor shall maintain protective covering in place and in good repair until removal is necessary.c. Deliver of specified items on order shall occur only when coordinated by MEDVAMC COR for delivery. d. Any government requested delayed delivery up to 30 days after initial negotiated delivery date, shall be at no additional cost to the Government.7. 0 DELIVERABLESa. Samples and Literature for Verification:For each type of exposed finish required, provide an actual sample, not less than three (3) inches square, of same thickness and material indicated for the work. Sample(s) shall be submitted within 10 business days ofaward. Literature and specifications shall be submitted within 10 business days.8.0 PROTECTION OF PROPERTYa. Contractor shall protect all items from damage. The Contractor shall take precaution against damage to the building(s), grounds and furnishings. The contractor shall repair or replace any items related to building(s) or grounds damaged accidentally or on purpose due to actions by the Contractor.b. The Contractor shall perform an inspection of the building(s) and grounds with the COR prior to commencing work to ensure that the Contractor shall be able to repair or replace any items, components, building(s) or grounds damaged due to negligence and/or actions taken by the Contractor. The source of all repairs beyond simple surface cleaning is the facility construction contractor (or appropriate subcontractor), so that building warranty is maintained. Concurrence from the VA Facilities Management POC and COR is required before the Contractor may perform any significant repair work. In all cases, repairs shall utilize materials of the same quality, size, texture, grade, and color to match adjacent existing work. 9.0 WARRANTY & REPAIRSa. All specified product(s) shall be under standard manufacture s warranties.b. Contractor must respond and provide a report for service and warranty repair request within 24 hours of notification for COR review and approval. COR will provide a review within 24 hours of receipt of report. Contractor must coordinate with the COR the service or warranty repair.10.0 PERIOD OF PERFORMANCE AND WORKING HOURS a. Furniture installation from contractor (HIS) shall be required between mid- January to late February 2020. Exact days will be coordinated according to furniture shipments to job site. Any government requested delayed delivery/installation up to 30 days after initial negotiated delivery/installation date, shall be at no additional cost to the Government. b. Duty hours will be 8:00 am 4:30 pm. The Government recognized US holidays are: New Year s Day, Martin Luther King, Jr. Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. If the holiday falls on Sunday, it is observed on Monday.The Government maintains the option to modify the normal work week, days and hours, as necessary for the accomplishment of VHA mission.11.0 PRODUCTGeneralAll finished surfaces shall be free of scratches, mars, dents, or blemishes, and withstand staining, scorching from heat of a cigarette, and exhibit to flaking,cracking, or loss or adhesion.Furnishings shall have smooth finishes with not hazardous projections, sharp corners, or detail, which can be hazardous and cause personal injury or damageto clothing.Reference the MTR s under IDIQ Contract for technical requirements of products below.
