This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued.This solicitation is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through FAC Number/Effective Date: 2005-101 / 10-26-2018.The Network Contracting Office 8-SAO East intends to award a Firm Fixed-Price Requirements Contract to provide the VISN 8 Sunshine Healthcare Network with commercial services, providing for all labor and services necessary to complete: Pick-up, Set-up, Inventory Management, disassembly, routine preventive/service/refurbishment maintenance, site assessment, disposal, storage and delivery service of Durable Medical Equipment (DME) and supplies to all beneficiaries serviced by VISN 8 s West Palm Beach and Miami VA Medical Centers and the associated outpatient clinics in accordance with the below Performance Work Statement (PWS) and attached Price Schedule. This procurement is being conducted under FAR Subpart 13 and simplified acquisition procedures apply. Period of performance is 01 January 2020 31 December 2025. The is a base year with four (4) one-year options requirement. The associated North American Industrial Classification System (NAICS) code for this procurement is 621610-Home Health Care Services, with a small business size standard of $15 million.This procurement is a TIERED Evaluation limited to Service Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB): This solicitation is being issued as a tiered evaluation for SDVOSB concerns, or in alternative, as a tiered evaluation for VOSB concerns. If award cannot be made, the solicitation will be cancelled, and the requirement resolicited.A.4 Vendor questions:� All questions regarding this RFQ must be in writing and may be sent by e mail to amber.milton@va.gov.�Questions must be received no later than Monday 12/02/2019 by 11:00 AM Eastern.� A.5 Answers to vendor questions:� Answers to vendor questions will be posted on FBO no later than Thursday 12/05/2019 by 02:30 PM Eastern.You are reminded that representatives from your company SHALL NOT contact any Bay Pines VA Health Care System staff to discuss this RFQ during this RFQ process. All questions and concerns regarding this RFQ shall be directed to Contracting Officer, Amber Milton at amber.milton@va.gov.A.6 Quote submission:� Offers are due on/or before of Thursday 12/12/2019 by 02:30 PM Eastern.� Quotes shall be sent by e mail only to the Contracting Officer, Amber Milton at amber.milton@va.gov. The Government will award a Firm Fixed-Price Requirements Contract to provide commercial services based on best value principles. Accordingly, award will be made resulting from this RFQ to the responsible and technically acceptable Offeror whose proposal provides the greatest overall value to the Government, price and other factors considered. This best value determination will be made by comparing the value of the differences in the technical qualifications of competing offers based on their strengths, weaknesses, and risks, with differences of their price to the Government. The following factors shall be used to evaluate offers: Price Technical Acceptability Technical and past performance, when combined, are more important than price. Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).The vendors shall submit the quote as identified in the INSTRUCTIONS TO QUOTERS, and all the required documentation requested in the Instructions to Quoters.
