- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for this action. Proposals are being requested, and a written solicitation document will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2019-06, 10-10-2019.This solicitation is issued as a Request for Proposal (RFP). Submit written proposals on 36C24820R0020. The North American Industrial Classification System (NAICS) code for this procurement is 621910, with a small business size standard of $16.5 million.This solicitation starts 11/07/2019 and will end 12/06/2019 at 4:30 PM EST.The purpose of this amendment is to update the Statement of Work and the SF1449 and provide the response to questions asked.The Network Contracting Office 8-RPO East intends to award a firm-fixed price contract for Advanced Life Support (ALS) and Basic Life Support (BLS) Ground Transportation Service, as identified within this Statement of Work (SOW) for beneficiaries of the NF/SG, LCVAMC, 619 S. Marion ave. Lake City, FL 32025 and, its associated healthcare facilities.All offerors shall be registered in System for Award Management (SAM) at https://www.sam.gov prior to award in accordance with the provisions at 52.212-3 Offeror Representations and Certifications Commercial Items, to be eligible for an award. Also, if you are a Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran-Owned Small Business, (VOSB), you must be registered in VIP (Vendor Information Pages) at: https://www.vip.vetbiz.gov/ prior to award.Contractors interested in providing these services should submit their quotes via email to Shirley.courtney@va.gov.
