- Page 2 of 2The Department of Veterans Affairs, Great Lakes Acquisition Center (GLAC) is searching for vendors with the capability of providing special mode transportation services to the beneficiaries of the following Community-Based Outpatient Clinics (CBOCs) that fall under the jurisdiction of the Clement J. Zablocki VA Medical Center in Milwaukee, WI:Milo C. Huemphner (Green Bay) CBOC Cleveland CBOC2851 University Ave. 1205 North Ave. Green Bay, WI 54311 Cleveland, WI 53015John H. Bradley (Appleton) CBOC10 Tri Park WayAppleton, WI 54914The Contractor shall furnish the necessary personnel and equipment to provide for twenty-four (24) hour 7 day a week wheelchair van and sedan transportation services to the beneficiaries of the above-listed CBOCs. The place of performance is variable, occurring within a 120-mile geodesic radius of the Green Bay CBOC. A majority of the orders (approximately 1,000 transports per year) will involve a pick-up or drop off address at one of the above-listed CBOCs. Occasional orders will involve other VA facilities within VISN 12 and non-VA medical facilities in the area. The North American Industry Classification System (NAICS) code for this procurement is 485991.All interested, responsible sources must submit a capability statement by Monday, December 9, 2019 to maureen.falaschi@va.gov to include the following: Company Experience in providing transportation services to medical patients, to include number of years and any examples of current contracts in place with government or commercial entities;Customer point of contact information for contracts listed in company experience along with a description of services provided under the contract (types of services provided, volume of trips per year, geographic service area covered; Size and type of fleet (sedans and vans rear loaders and side loaders);Number of drivers currently employed;Ability to provide immediate access and communication between customers and their personnel 24 hours a day, 7 days a week;Example(s) of quality control programs and procedures currently utilized;DUNS number, organization name, organization address, point of contact, business size under NAICS 485991, and socioeconomic status;GSA contract number (if a GSA contract holder);Licensing for states and state requirements.This RFI is issued solely for information and planning purposes to determine availability of qualified vendors and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly.
