PHILIPS ISCV UPGRADE FROM XCELERA ECHO SYSTEMStatement of Work (SOW)GENERALEl Paso Veterans Affairs Health Care System (EPVAHCS) requires an upgrade that will allow the El Paso VA Cardiology Service to transition to a web-based workstation thus freeing up current Reading rooms. This will be a twofold benefit to Patient care. First and most important is that the cardiologists will have full web accessibility at their own workstations and on their VA laptops from home or away from office; this will expedite turnaround times for final report results. Second, it will allow Cardiology Service to reclaim the Philips Holter monitoring system into a full connectivity test procedure; it is currently a stand-alone non-networked system that creates delay and an additional scan process for final report upload into the VA EMR.DESCRIPTION OF WORK2.1. Adding a new web server to current XCELERA/IBE server configuration and transition to the latest Intellispace Cardiovascular software release. This upgrade consists of new servers, ISCV Production, Web Production, ISCV Web Test and four new licenses to Windows server 2012 R2 in addition to SQL Server 2014.2.2. General requirements:Minimum requirement Included in this upgrade transition is there is also a replacement of the physical servers that support the application with new physical servers, including new operating system and SQL server licensing and SQL user and Client licensing. The test environment will include a single server that will serve as the test platform for the database server and the web serverFinally, there are eight hours of onsite professional services work effort, dedicated to theinstallation and configuration of servers that are included in this proposal. A (CPACS) application upgrade to our latest available software release of IntelliSpaceThere will be training and support: Applications Training, Remote 40 hrs. Go Live Support, Onsite 16 hrs. System Administration Training, Remote 4 hrs. Follow up Training, Remote 8 hrs. Advanced Analytics Implementation & Training, Remote 6 hrs. Finally, there are eight hours of onsite professional services work effort, dedicated to the installation and configuration of servers that are included in this upgrade.2.3. TRAINING:Training shall include minimum one (1) day on-site training for staff, Technician staff and Biomedical staff. It must include Specialized Training [travel & lodging included] for Administrator and Super User Level for Biomedical Specialist and designated Technician at the Vendor primary training facility.2.4. DELIVERY:Delivery shall be made to:El Paso VA Health Care System (EVAHCS)5001 N. PiedrasEl Paso, TX 79930FOB Destination2.5. The Contractor is expected to respond with 24 hours of the maintenance call. Reports or documentation is required to be submitted after repairs.3. WORK HOURS:3.1. Normal Work Hours: The service schedule shall be developed between the contractor and Contractor s Representative (COR) prior to any service being performed.3.2. The following is a list of U.S. Government holidays. PERSONNEL4.1. The contractor shall provide in writing the personnel name and phone number within (10) ten calendar days of the award of the contract. Personnel shall be a qualified and experienced to oversee the personnel assigned to perform the installation and maintenance services. The contractor s personnel shall correspond with the COR on a regular basis to discuss any problems that the contractor or contractor s personnel may be experiencing during the performance of this contract. Unresolved problems shall be referred to the Contracting Officer for resolution.4.2. Contractor Service Personnel (CSP). All subcontractors performing work for primary contractor shall meet all specifications and standards that apply to CSP under this agreement. CSP shall maintain clean and neat appearance and shall wear an identification badge at all times when performing services at the Government site. Identification badges shall be worn in a clearly visible area of the outer garment. The COR shall furnish this badge. Due to conflict of interest, the contractor shall not employ a current DOD employee, military or civilian to provide services under this contract.4.3. Government point of contact (POC). The COR shall be the Government s POC. If required, the COR shall be designated in writing to the Contractor and the scope of authority shall be set forth therein. Contractor shall respond only to calls from COR or a designated representative from the Medical Center.5. EQUIPMENT OWNERSHIP.5.1. Title to equipment shall remain with the contractor until installed and established. After completion, a satisfactory inventory and inspection is completed by Contractor, COR and Maintenance personnel. Upon approved inspection, title, equipment, accessories and ownership shall be released to EL Paso Veterans Affairs Health Care System (ELPVAHCS).6. LIMITED WARRANTY.6.1. All equipment listed to the attached quote, shall be fit and sufficient for the purpose intended as set forth in the user manuals; and merchantable, of good quality and free from defects in materials or workmanship; for a period of one (1) year from the date of the first invoice under this agreement.
