Description
The purpose of this sources sought is to conduct market research to support the procurement of Fracture Tables for the Gallup Indian Medical Center (GIMC) in Gallup, New Mexico. This notice serves to survey the market. This notice allows potential contractors/vendors to submit a non-binding statement of interest and documentation demonstrating their capability. The sources sought effort is intended to assess industry capabilities and develop/support the intended direction of acquisition planning. The GIMC Acquisition is requesting responses from qualified business concerns; the qualifying NAICS code for this effort is 339113 - Surgical Appliance and Supplies Manufacturing.This sources sought is to survey the socio-economic businesses available that the Government may consider during the development of the Acquisition strategy.Background: GIMC is a 62-bed inpatient which provides clinical specialties inclusive of Internal Medicine, Cardiology , Anesthesia , OB/GYN, General Surgery , Orthopedics , Ophthalmology , ENT, Radiology Pathology , Pediatrics , Podiatry, Psychiatry , Emergency Medicine, and Urology. The workload at GIMC is one of the largest in the Indian Health Service with 250,000 outpatient encounters and 5,800 inpatient admissions annually. GIMC has the largest staff of all NAIHS facilities and is considered a Level 3 trauma hospital. INSTRUCTIONS: Your response shall include the following:1) Submit a brief description, two-three (2-3) pages (including cover letter) that demonstrates how your company can provide the requested equipment and services.2) Include past performance in providing the requested equipment and services to the Indian Health Service, other Government (Federal or State) agency, or for a private facility, within the past three (3) years. Include contract number, point of contact telephone number or e-mail address.3) Include the address of your corporate office and local field office and technical services available.4) Please indicate whether supplies and services are available under contract with GSA Federal Supply Schedule, DOD (Veteran National Acquisition Center) and, if so, provide the contract or indicate if supply and services are open market. 5) Provide BUSINESS SIZE AND SOCIO-ECONOMIC STATUS; (a) Indicate whether your business is large or Small, Native-American owned, certified Woman, Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB), 8(a), or HUBZone.6) Include the DUNS number of your firm. State whether your firm is registered with the System for Award Management (SAM) at http://www.sam.gov and/or the VetBiz Registry at http://vip.vetbiz.gov.Response is due by 11:00 AM (MSD), Tuesday, December 31, 2019.
