2VA Pittsburgh Healthcare Systems (VAPHS)Performance Work Statement (PWS)Bariatric Bed, Wheelchair and Shower/ Commode Chair RentalsSERVICES AND QUANTIES /PRICES/COSTS- Furnish rental specialized medical beds and bariatric beds for veteran s acute care and long-term use for VAPHS at both the University Drive and HJ Heinz Divisions. Items need to be available 24 hours, 7 days a week including holidays within 2-6 hours of call-in time to contractor. Patient safety is one of the primary goals for VAPHS. A recent action item forwarded by the Associate Director of Patient Care Services, directs the use of rental beds which perform all functions from the lowest position possible to minimize fall injuries. Pricing must include all services and requirements stated in the solicitation. The contract will be for a base plus four (4) option years to exercise at the discretion of the VA. All quantities listed herein are estimated only. The Government is not obligated to any specified amount of services but shall be obligated to provide payment for all services performed in accordance with (IAW) the contractual terms and conditions.2. DESCRIPTION/SPECIFICATIONS/REQUIREMENTS 2.1 SERVICES: Contractor shall deliver, repair and exchange the required medical deliverables directly to patient care Units within a reasonable time of between 2 to 6 hours with an average of four hours within normal business hours (8a-5p, Monday through Friday). For calls received outside this time, add one hour. If for any reason any deliverable cannot be delivered within the scheduled time of 2 to 6 hours, the contractor is required to call and cite reasons for delay to the contact person (Program POC) and give a time when product will be delivered.24 hours/7 days a week Care Center availability to place orders or report service issues.Upon discharge of patient or no longer needing the deliverable, notification will be given to contractor to pick-up used product.Billing for the item(s) will end at the time the contractor is notified.Contractor is required to pick-up equipment within 36 hours of the call.VAPHS has both long-term and acute care facilities, there are occasions when patients are transferred between these facilities. When this happens VAPHS enterostomal nurses will analyze each case to determine if the bed should be held for the patient s return.This will eliminate delivery wait time for the patient s return to long -term care facility or medical units after treatment. The contractor will be notified to stop billing if the bed is being placed on-hold. The contractor will also be notified when the hold is lifted, and billing should continue. VAPHS will not place holds for less than 1 day, or more than 7 days. Only assigned nursing personnel or assigned administrative personnel can order specialty equipment and place holds either by telephone or online ordering.Upon discharge of patient, or when no longer needed; the same assigned personnel are to call contractor for pick-up of used medical deliverable.2.2 DELIVERABLES: Charges begin to incur when the deliverable is delivered on-site and charges end when the contractor is called for deliverable to be picked-up. The full day charge may not apply if the bed was not in possession for the entire period. Charges may incur for only one bed per patient at a time regardless if the contractor leaves their equipment on-site. The services to be performed by the Contractor will be monitored by the Contracting Officer Representative (COR).3. CONTRACTOR RESPONSIBILITIES/TRAINING REQUIREMENTS 3.1 TRAINING/SECURITY: The Contractor shall remove any employee from the performance of this contract within five (5) workdays of receiving notice from the Contracting Officer that the employee's performance is unsatisfactory. All Contractor employees are subject to immediate removal from performance of this contract when they are involved in a violation of the law, VA security, confidentiality requirements and/or other disciplinary reasons. Contractor employees are required to complete the online training classes within TMS entitled: VA Privacy and Information Security Awareness and Rules of Behavior course number VA 10176 HIPAA and Privacy Training course number VA 10203. Other training may be added or deleted as policies and guidance from higher authority is received. A Certificate of successful completion will be generated for each course. Copies of the certificates shall be maintained by the vendor and can be obtained from the COR; if the contract employee cannot print them. Completion of these training courses is required on an annual basis based on the VA s fiscal year. Contractor employees are required to have a VA issued ID (PIV Card) to receive access to VAPHS facilities. To receive a VA issued ID (PIV card) contract employees will be fingerprinted and scheduled for photo and issuance of the card.3.2 TRAINING OF CLINICAL STAFF: will be required if vendor product is selected with hands-on and e-learning modalities on the following topics: Installation and set-up of each placement.Patient transfer assistance at time of placement.Disinfecting and cleaning between each placement and periodic cleaning as needed.Ongoing user training and in-servicing as needed.Pre-installation maintenance and electrical safety checks between each patient use. Clinical staff to help to:recommend the right frames and surfaces in all areas of patient carecreate discharge and ICU transfer protocols to reduce readmission ratesparticipate in clinical rounds when requested.DELIVERABLES and SPECIFICATIONS:Air fluidized therapy bed assembly with low air loss therapy to prevent and treat pressure injuries that specifically targets the sacrum, ischium, trochanter and heels were wounds are likely to occur. Heinz Units:Ground South, 1N, 1S, 2A, 2B, & 3AORDERS: Request for services will be made by telephone or via internet by the Enterostomal Therapy Nurse, Patient Care Coordinator or nursing unit management teams. The contractor will be informed what commodity is needed (per the contract line items), what room and location it is going to be used in, name of Veteran, and when it is needed.The Contractor s employee will report to the unit nurse on duty but not before checking into the VA police department upon arrival. The commodity will be placed in the Veteran s room under the direction of nursing. The contractor will leave a delivery ticket with the unit nurse and the billing will begin once the beds or other items are in the room and in use.Whenever the bed or other commodity is no longer needed for Veteran, the Contractor will be notified to pick up the bed. Billing will stop at the time the Contractor is contacted that the bed or other commodity is no longer needed.Invoices shall be certified by the Certifying Official. Payment will be made upon certification of invoice. Invoicing will be submitted monthly in-arrears. All invoices from the contractor shall be submitted electronically through Tungsten Network.
