Page 8 of 8COMBINED SYNOPSIS/SOLICITATIONPHARMACY WALL TO WALL INVENTORY SERVICE AT THE NORTHPORT VA MEDICAL CENTER (i) This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6 Streamlined Procedures for Evaluation and Solicitation for Commercial Items and FAR part 13 - Simplified Acquisition Procedures , as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.(ii) The solicitation number 36C24220Q0140 is issued as a request for quote (RFQ) and will be submitted on through https://beta.sam.gov/. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-88-1.(iv) This procurement is being issued as 100% Small Business Set Aside Open Market Competition. The North American Industry Classification System (NAICS) code is 561990, All Other Support Services with a small business size standard of $12 million. (v) The Contractor shall provide pricing for Pharmacy Wall to Wall Inventory Service. The following factors shall be used to evaluate offers:Technical: The offeror shall describe in its technical solution how it plans to meet the requirement in the statement of work. The offeror shall provide details of specific methods, techniques, and solutions for completing the requirement, including areas such as technical approach, technical expertise (including experience and qualifications), and overall benefit to the Government. Technical evaluation factors consist of three (3) sub-factors:Certification:� The offeror shall provide proof of certification in pharmaceutical count methodologies (including unit dose/per pill, tenths and half/full), handling of medications and HIPAA regulations for all inventory auditors working on-site.Software Technology:� The offeror shall demonstrate its use of software technology to provide the highest level of accuracy and efficiency.Reporting:� The offeror shall demonstrate its ability to provide reports in 24-72 hours with same day option. Technical should not exceed more than 10 pages.Past Performance: A review of the offeror s past performance will assess whether the Contractor has the ability to successfully accomplish the elements outlined in the statement of work.� The offeror is required to provide 3-4 references which will be evaluated based on the relevancy of the information, the source of the information, and general trends in the Contractor s performance. The offeror past performance shall be similar in size, scope, and complexity as required by this SOW, include technical point of contacts for each project. Past performance projects can be for previous or ongoing task orders, work assignments, or contracts. The Government may contact sources of past performance and may obtain past performance from other sources such as Government databases.Price: Will be determined as fair and reasonable per FAR part 13.106-3.Options. The Government will evaluate offers for award purposes by adding the total price for alloptions to the total price for the basic requirement. The Government may determine that an offer isunacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate theGovernment to exercise the option(s).NOTE: Except when it is determined in accordance with FAR 17.206(b) not to be in the Government's best interests, the Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. This includes options under FAR 52.217-8, Option to Extend Services, which applies to this solicitation. Evaluation of options under FAR 52.217-8 will beaccomplished by using the prices offered for the last option period to determine the price for a 6-monthoption period, which will be added to the base and other option years to arrive at the total price.Evaluation of options will not obligate the Government to exercise the option(s).
