36C25020Q0192SPECIAL NOTICENOTICE OF INTENT TO SOLE SOURCEHausted Stretcher Eye Carts The Cleveland Department of Veteran Affairs Medical Center, 10701 East Blvd Cleveland, OH 44106 intends to solicit and negotiate with only one source under the authority of FAR 13.1061(b)(1)-Contracting officers may solicit from one source if the contracting officer determines that the circumstances of the contract action deem only one source reasonably available. The NAICS code is 339113 with a small business size standard of 750. The VA intends to solicit and negotiate with GF Health Products, Inc. 1 Graham Field Way Atlanta, GA 30340. VA Northeast Ohio Healthcare System, Cleveland, Ohio is requesting motorized eye stretcher carts (qty 5). There is a need for 5 Hausted motorized eye stretcher carts to replace the current poor functioning stretcher eye carts. They no longer meet patient needs and the needs of surgeons during surgery, which increase the risk for staff injury due to lifting and manually moving patients and do not comply with the current Safe Patient Handling and Mobility (SPHM) Directive. The stretcher carts shall contain contoured head rests. Two of the stretcher eye carts shall have wrist rests for Outpatient Ambulatory Surgery Surgeons to use during eye surgery. These motorized eye stretcher carts are required for positioning and transporting patients, pre/during/post-surgery to minimize staff injury and patient injury (particularly prevention shoulder and back injuries).Motorized eye stretcher carts will be utilized Outpatient Ambulatory Surgery for patients that need specialize eye procedures that require precise movement of the head piece and body positioning to prevent injury to patient, ensure proper body mechanics of caregiver and allow for ease of use for physician/anesthesia during surgery. This requested work is in direct compliance with Medical Center Policy 118-034, (Safe Patient Handling & Mobility Program), the goal of VHA DIRECTIVE 1611 (Dated: March 23, 2018) (Safe Patient Handling and Mobility Program) and Facility Design and GLA Policy 00-10B-118-130-39 which protects caregivers and patients from injuries due to patient handling and movement . This is a direct replacement/upgrade, and standardization of equipment type, and brand is important for staff education and patient care, as well as compliance with medical center safe patient handling policy.Other persons may identify their interest and capability to respond to this requirement by Friday December 13, 2019 at 2PM EST. Such capabilities information will be used solely for the purpose of determining whether to conduct this procurement on a competitive basis. �All information shall be furnished at no cost or obligation to the Government.� A determination by the Government not to open the requirement to competition based upon responses to this notice is solely within the discretion of the Government.
