Description
STATEMENT OF WORK LAB COATSGENERAL:Furnish all labor, supervision, materials, and equipment necessary to provide lab coat rental for the Philadelphia VA. The vendor will barcode and RFID chip each coat and scan clean and soiled coats and will provide detailed reporting on usage by each authorized lab coat user. The vendor will deliver lab coats to each individual, department, or user. As well as collect all soiled coats while adhering to all infection control practices.WORK HOURS:Normal Work Hours: The service schedule will be developed between the contractor and the contractor representative, Ralph Johnson (COR) prior to any service being performed. Generally, work will be performed prior to the start of the medical center s workday. So that trucks and pickups will not interfere with the facility s normal functions.National Holidays: The ten holidays observed by the federal government are: New Year s Day, Martin Luther King s Birthday, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran s Day, Thanksgiving Day, and Christmas Day and any other day specifically declared by the President of the United States to be a national holiday.SPECIFICATIONS:Contractor shall provide contractor owned lab coats for all designated employees specified in this contract. The coats shall be of quality and condition as determined by the VA. Coats shall be free of stain, tears, and holes. Coats with minor tears and stains are not acceptable. The company name and logo will be clearly labeled on all lab coats. The lab coats shall be 100% cotton fabric with knot buttons.Each coat will be clearly labeled and barcoded and have a RFID chip inserted in the hem. All soiled coats will be picked up, laundered, and delivered to each individual department on a weekly basis. QUALITY ASSURANCE:The vendor and the Philadelphia VA representative will inspect coats on a regular basis and replace any coats determined to be unacceptable. CONTRACTOR PERSONNEL BADGES AND PARKING:The contractor shall provide the COR with a list of contractor employees expected to enter the buildings to pick up confidential documents. While on VA premises, all contract personnel shall comply with the rules, regulations, and procedure governing the conduct of personnel and the operation of the facility. An access bade will be given to the contractor s employee upon entrance into the VA buildings. The contractor employee must safeguard the access bade and immediately report any lost, stole, or destroyed badges to the COR. All contract personnel must properly display their access badges. Access badge(s) must be worn at or above the waist (facing forward). The contractor s employees must return the access badge(s) to the COR or designee at the end of each pick up process. The contractor shall be required to comply with all security policies/requirements of (your facility). All security policies/requirements must be met, and employees cleared prior to the contractor performing work under this contract. Employees that cannot meet the security and clearance requirements shall not be allowed to perform work under this contract.It is the responsibility of the contractor s personnel to park in the appropriate designated parking areas. Parking information shall be coordinated with each facility COR.Philadelphia does not validate or make reimbursement for parking violations of the contractor s personnel under any circumstance.INTERFERENCE TO NORMAL FUNCTION:Contractor may be required to interrupt their work at anytime so as not ot interfere with the normal function of the facility, including utility services, fire protection systems, and passage of facility patients, personnel, equipment, and carts.In the event of an emergency, contractor services may be stopped and rescheduled at no additional cost to the government.Contractor personnel shall inform the COR or the designee of the need to gain access to secured areas. If access is required to secure areas, prearranged scheduling will be made with COR or designee. LAB COAT REQUIREMENTSThe vendor will provide 100% cotton lab coats in the size and quantity to the designated employees at their locations listed on attachment A. Provide 100% cotton full length lab coats with knot buttons.Barcode and RFID each coat.Deliver clean coats to each department and collect soiled coats.Scan coats on delivery.Collect soiled coats and scan each coat out. Provide detailed reporting on individual usage.Provide weekly invoicing with monthly summary invoice.
