NOTICE OF INTENT TO AWARD SOLE SOURCETitle: CISCOR Patient/Staff Duress System Salt Lake City, UTThe Department of Veterans Affairs, Network Contracting Office 19 intends to award a sole source contract with the authority of FAR Part 6.301-1, Only One Responsible Source to CISCOR, Inc. 126 W Main St. Norman, OK 73069-1307. This notice is for informational purposes only and is not a request for quotation. Responses to this notice must demonstrate clear and convincing evidence that competition would be advantageous to the Government, would not be cost or time prohibitive, and would not hinder the mission objective of the Department of Veterans Affairs, NCO 19. Questions or comments to this Notice of Intent should be directed to Contract Specialist Susan Dela Cruz, email address: susan.delacruz@va.gov and response must be received by December 26, 2019 and 1200/MST.Statement of WorkCISCOR Patient/Staff Duress SystemSalt Lake City Health Care System (SLCHCS)500 Foothill DriveSalt Lake City, UTThe SLCHCS Biomedical Engineering Department has a requirement for a Full-Service Maintenance Agreement for the CISCOR Patient/Staff Duress System to support both Patient Care and Staff safety. BackgroundThe VA Salt Lake City Health Care System (SLCHCS) over the last 6 years has installed, utilized and expanded the Patient/Staff Duress CISCOR System that now requires full maintenance support. No on-site personnel are trained to support this system. The system is now installed in Bldg. 1 and Bldg. 14 Main Medical Campus Hospital, Bldg. 3 IPU (Inpatient Mental Health Unit, Bldg. 3-SRRTP (In-Patient Treatment), Bldg. 16 Outpatient Mental Health. This system is the main Police alert alarm system for the Patients and Staff during any emergency call. This system includes all emergency alarm buttons at every desk in every area of the main Bldg. 1 and Bldg. 14 Medical Treatment areas (Panic Alarms). Scope of WorkFree Unlimited Phone Support 8:00 5:00 Monday Friday/Non-Holidays20 Free After Hour Calls all calls other calls will be billed at $50 per event One annual on-site visit and full system evaluation and functionality testing to include failover system test. Plan includes emergency service for a CISCOR technician to be on-site for up to a week24/7/365 Dial-in Troubleshooting Free Telephone Training during normal business hoursFree OneSource Software UpdatesEquipment Replacement: During the contract period CISCOR will replace any failed equipment specified below normal wear and tear is acceptable. Abuse of equipment is not covered. All defective equipment will be returned to CISCOR. Shipping fees not included. Annual On-Site system Inspection and function testing One of the CISCOR technicians will be on-site at the facility once a year to complete a full system inspection to ensure that everything is functioning at peak efficiency. This includes running our wireless diagnostic software which analyzes every aspect of our wireless system identifying potential problem areas. Other support operations and analysis operations will include: Identifying database configuration errorsLooking for alarm and event history anomalies Checking for excessive alary response timesRunning wireless system diagnostic and analysis softwareUpdating the operating systemChecking Windows Event Log Errors Ensure failover server is functionEstimated Period of Performance 1/15/2020 1/14/2021 with four (4) one-year option period.Information SecurityPer VA Handbook 6500.6 Appendix B: Due to the threat of data breach, compromise or loss of information that resides on either VA-owned or contractor-owned systems, and to comply with Federal laws and regulations, VA has developed an Information and Information Technology Security clause to be used when VA sensitive information is accessed, used, stored, generated, transmitted, or exchanged by and between VA and a contractor, subcontractor or a third party in any format (e.g., paper, microfiche, electronic or magnetic portable media).In solicitations and contracts where VA Sensitive Information or Information Technology will be accessed or utilized, the CO shall insert the clause found at 852.273-75, Security Requirements for Unclassified Information Technology Resources.The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information and information system security as delineated in this contract.Per VA Handbook 6500.6 Appendix C: Information made available to the contractor or subcontractor by VA for the performance or administration of this contract or information developed by the contractor/subcontractor in performance or administration of the contract shall be used only for those purposes and shall not be used in any other way without the prior written agreement of the VA. This clause expressly limits the contractor/subcontractor's rights to use data as described in Rights in Data - General, FAR 52.227-14(d) (1).(End of Page)
