- Department of Veterans Affairs, Network Contracting Office 2, NY Harbor Healthcare System is issuing this Notice of Intent to Sole Source in order to identify any potential sources that have the resources and capabilities to provide a comprehensive service for Amalgam Separators and Recycling Services for four different locations. SERVICES REQUIRED:Contractor shall provide annual Amalgam Separator and Recycling Services, changeout Service of BU Amalgam Separator and Amalgam Recycling Kits for the New Jersey Veterans Affairs Medical Center at four (4) different Dental Locations. Period of Performance will be one (1) year from date of award of contract with provision for four (4) Option Years. The NAICS code for these services is 562112 (Hazardous Waste Collection)INTERESTED & CAPABLE RESPONSES:NCO 2, NY Harbor Healthcare System is seeking responses from businesses that are interested in this procurement and consider themselves to have the resources, rights, and capabilities necessary to provide these services. Please provide all of the following in your response:Business name, business type, socio-economic status (e.g., Veteran-Owned, Woman-Owned, Disadvantaged Small Business, 8(a), etc.), person of contact (including telephone number & email address), GSA Contract number if applicable, and DUNS number. Please also include the North American Industry Classification System (NAICS) Code for which you propose to do provide the service under for verification of your size standard. A Capability Statement which provides detailed, specific information that demonstrates understanding of the tasks required and capability to perform the tasks as stated in the attached Statement of Work. Failure to tailor capability statement to this specific requirement may result in your company being determined non-responsive. Responses must be submitted by 3:00 PM EST December 20, 2019. VA assumes no responsibility for any costs incurred associated with the preparation of responses submitted for this notice.STATEMENT OF WORKBackground: Department of Veterans Affairs New Jersey Health Care System is seeking annual services for Amalgam Separator Replacement Kits and Recycling Service at these locations (East Orange, Lyons and Brick). Dental offices discharge mercury present in amalgam used for fillings. Amalgam separators are a practical, affordable and readily available technology for capturing mercury and other metals before they are discharged into sewers. Amalgam separators remove amalgam particles from the wastewater to reduce the amount of amalgam entering the sewage system.�� Scope of Work: Contractor is required to furnish labor, parts and equipment for Amalgam Separator Replacement Kits & Recycling Services and pre-paid shipping To and From each location. Contractor shall provide certification of completion of services. Contractor shall provide a Final report specifying how much Amalgam was collected.
