RFQ#36C26220Q0216PROJECT TITLE: ICP MEDICAL REFRESH DISPOSABLE PATIENT CURTAINS FOR VETERANS AFFAIRS SAN DIEGO HEALTHCARE SYSTEMThis is a combined synopsis/solicitation set-aside for Service Disable Veteran Owned Small Businesses (SDVOSB) and Veteran Owned Small Businesses (VOSB) for commercial supplies/services prepared in accordance with the format in Subpart 12.6, as supplemented with the additional information included in this notice. The combined synopsis/solicitation is set-aside for qualified SDVOSB s under NAICS 314120 or related to. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a request for quotes under solicitation number: 36C26220Q0216. The Department of Veterans Affairs San Diego Healthcare System, San Diego, CA 92161 has a requirement for the ICP Medical Refresh Disposable Patient Curtains. The successful contractor shall furnish all supplies/services at the VA San Diego Healthcare System 3350 La Jolla Village Drive, Warehouse Bldg. 1, San Diego, CA 92161. Any items quoted as equal not confirming to the requirements of clause 52.211-6 will be considered non- responsive and not considered for award. Vendor Requirements: Vendor shall make four (4) deliveries throughout the period of performance.Vendor shall deliver 25 boxes every two weeks.Each box shall contain a quantity of 10 privacy curtains.Vendor shall provide standard manufacture warranty on the product starting from the first day of delivery. The pricing terms and conditions of the quotation shall be valid for at least (90) days unless a longer time is stated in the quote.DESCRIPTION OF SUPPLIES FOR THE VA SAN DIEGO HEALTHCARE SYSTEMMedical Refresh Disposable Patient Privacy Curtains1. The purpose of this Statement of Work (SOW) is to outline the tasks and responsibilities of the successful offerors in relation to the VA San Diego. 2. SCOPE Vendor is to provide Medical Refresh Disposable Patient Privacy Curtains as described below to the VA San Diego Healthcare System. ITEM NUMBER: ICP-6100TF DESCRIPTION: ICP Medical Refresh Disposable Patient Privacy Curtains, Teal FlowerQUANTITY: 1003. PLACE OF DELIVERY All supplies must be delivered to the facility s Shipping and Receiving Department located at 3350 La Jolla Village Drive, San Diego, CA 92161. 4. INSPECTION AND ACCEPTANCE The VA technical Point of Contact is responsible for performing inspections necessary to substantiate that the supplies furnished under this contract are not damaged and in good working condition. Any items found to be damaged and/or not in working condition, would be returned to the contractor and must be replaced in a timely manner. 5. WARRANTY REQUIREMENTS Vendor is to provide standard warranty for these supplies. The Contractor shall adhere to all federal and state laws and regulations in effect during the term of this contract. The Government intends to award a firm-fixed-price contract.
