The U.S. Army Contracting Command � Rock Island (ACC-RI) hereby issues this combined synopsis/solicitation Request for Quote (RFQ) as a Firm-Fixed Price, 100% Small Business set-aside in accordance with (IAW) FAR 19.502-2(b)(1) and IAW the terms and conditions listed herein. This RFQ is for commercial Solid Waste Removal Services in support of the Pine Bluff Arsenal Directorate of Public Works (PBA-DPW), in accordance with ""Attachment 0001-Performance Work Statement (PWS)"". 2. The following NAICS code and small business size standard apply: NAICS: 562111 Size Standard: $41,500,000.00. 3. ACC-RI anticipates awarding a single firm-fixed price contract with a one year base and 4 option years. 4. Solid Waste Collection will begin 15 days after award at Pine Bluff Arsenal, Pine Bluff, Arkansas. 5. In response to this solicitation, vendors shall submit the following: a. Attachment 0002 � Pricing Matrix to submit its price quote. To be considered a responsive quote, vendors shall include a price for each field in the Pricing Matrix, including each option year and the total price. This document shall also include an affirmative statement that the Contractor can conform to the terms as listed in this solicitation with a duly authorized signature of a Company Officer that has authority to commit the vendor to a USG contract. b. Technical Volume � The Technical Volume shall include the following: i. Equipment Listing. Provide a complete list of vehicles, equipment, and tools to be used in the performance of the contract. At a minimum this list shall include the make, model and year of each piece of equipment. All vehicles used in the performance of this contract shall comply with PWS 4.3.2. ii. Quality Control Plan (QCP). Contractor shall provide its QCP. This plan shall be describe the contractor�s plan to ensure all services are performed in accordance with the PWS. The plan shall identify, prevent, correct, and ensure non-recurrence of defective services. 6. Quotations will be evaluated as follows: A single Contract will be awarded in accordance with terms and conditions set forth below to the lowest price, technically acceptable, responsive, and responsible contractor whose price is determined to be fair and reasonable. To be considered for award, a contractor must have an ACCEPTABLE rating for the Equipment Listing AND the QCP. If either one of these items is rated UNACCEPTABLE, the Contractor may not be considered for award, regardless of price quoted. a. The Contractor�s quote will be reviewed to ensure a complete Attachment 0002 is submitted timely. Contractor shall complete all YELLOW cells on the Line Item Pricing tab. No other changes shall be made to the spreadsheet. The Total Evaluated Price will be comprised of the ""Yearly Total All Line Items"" for the base year and all option years. ACC-RI may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the options. b. The Contractor�s quote will be reviewed to ensure a complete Equipment Listing is submitted with its quote. The definition of Acceptable & Not Acceptable are below. Acceptable: Contractor has submitted a complete listing of vehicles, equipment, and tools to be used in the performance of the contract. The Contractor�s quote will be reviewed to ensure an effective QCP is included with its quote. The definition of Acceptable & Not Acceptable are below. Acceptable: The plan adequately describes the Contractor�s plan to ensure all services are performed IAW with the PWS. The plan shall also identify how the contractor will identify, prevent, correct and ensure non-recurrence of defective services. Not Acceptable: The plan does not adequately describe how the contractor will ensure all services are performed IAW the PWS. The plan does not describe how the Contractor will identify, prevent and endure non-recurrence of defective services. 7. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
