- The purpose of Amendment 1 is to update the solicitation documents�as a result of questions received. Attached is a conformed copy of the solicitation (70SBUR20Q00000018 Amendment 1), a conformed copy of the solicitation with track changes (70SBUR20Q00000018 Amendment 1 - Track Changes), an updated copy of the pricing spreadsheet (70SBUR20Q00000018 Attachment C - Pricing Spreadsheet Amendment 1), and a copy of questions and answers (70SBUR20Q00000018 Amendment 1 Q+A). This is a combined synopsis/solicitation for Civil Surgeon Credential Verification�in accordance with FAR 12.603 with the synopsis requirements of FAR 5.207. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) Solicitation number 70SBUR20Q00000018 is being issued as a request for quotation (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-01. (iv) This is an open market unrestricted solicitation. The associated NAICS code is 541990 � All Other Professional, Scientific and Technical Services with a small business size standard of $16.5M. (v) See Attachment C � Pricing Spreadsheet for a list of line item number(s) and items, quantities, and units of measure (including options). (vi) This requirement is for Civil Surgeon medical license verification and reporting, Civil Surgeon medical certification verification and reporting, and derogatory information attained through the National Practitioner Data Bank and reporting as outlined in the Statement of Work. (vii) Period of performance for the base period will be 1/12/2020 � 1/11/2021 and shall include four (4) one (1) year option period that if exercised shall extend the period of performance to a maximum of 60 months. Delivery and acceptance of deliverables shall be in accordance with the Statement of Work. (viii) The provision at 52.212-1, Instructions to Offerors-Commercial Items applies to this acquisition as does the addendum (Pages 28-30 of RFQ 70SBUR20Q00000018). � (ix) The provision at 52.212-2, Instructions to Offerors-Commercial, applies to this acquisition. The specific evaluation criteria outlined in paragraph (a) of 52.212-2 are: Technical Acceptability and Price. (x) A complete copy of the provision 52.212-3, Offeror Reps and Certifications-Commercial Items shall be included with the quote. (xi) The clause 52.212-4, Contract Terms and Conditions- Commercial Items applies to this acquisition as does the addendum regarding Invoicing Instructions (Pages 12-13 of RFQ 70SBUR20Q00000018). (xii) The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders- Commercial Items applies to this acquisition as well as the additional FAR clauses cited in FAR 52.212-5: (See RFQ 70SBUR20Q00000018). (xiii) RFQ 70SBUR20Q00000018 is being issued with the intent to make one (1) award. Award will be based on a Lowest-Price Technically-Acceptable quote. Submission of a complete quote constitutes affirmation of your ability to provide Civil Surgeon credential verifications as outlined in the attached solicitation documents. Amendments to this solicitation will be publicized in the same manner as the initial synopsis and solicitation. The Government is requesting maximum additional discounts beyond standard commercial pricing for this the requirement. The quote shall display any discount provided by the contractor and should represent the best possible price that the contractor can provide. In accordance with FAR 22.1009-4(a): (1) The place of performance is unknown and will be performed at the awarded vendor�s place of business. (2) The Contracting Officer has not identified any possible places of performance. (3) The Contracting Officer will obtain additional wage determinations for additional places or areas of performance if asked to do so in writing. (4) Potential awardee must notify the Contracting Officer of additional places of performance within seven (7) business days after award.
