- This is a combined synopsis/solicitation to procure a staffing agency able to fill one Flight and Operational Medicine Clinic (FOMC) Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to provide all management, tools, supplies equipment, and labor necessary to operate and maintain support for the Flight Medicine Clinic at the 18th Medical Group, Kadena Air Base. Please review RFQ�#FA527020QB002 and any attachments in full. (i) This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.� (ii) The solicitation, FA527020QB002,�Flight and Operational Medicine Clinical (FOMC) Nurse, is being issued as a request for quotation (RFQ).� Amendment 1 -�Response to Questions was posted 23DEC19.�
