- THIS REQUEST FOR INFORMATION (RFI) 36C26220Q0286 -IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. THE SUBMISSION OF PRICING, CAPABILITIES FOR PLANNING PURPOSES, AND OTHER MARKET INFORMATION IS HIGHLY ENCOURAGED AND ALLOWED UNDER THIS RFI IN ACCORDANCE WITH (IAW) FAR 15.201(e). SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a source sought/RFI for market research only for Small Businesses (SB). The purpose of this source sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to NAICS 339112 (size standard is 1,000 Employees). Responses to this source sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this source sought, a solicitation announcement may be published. Responses to this source sought synopsis are not considered adequate responses for a solicitation announcement. The Department of Veterans Affairs (VA), VISN 22 Network Contracting Office, is seeking sources that can provide 1 EA Power Surgical Table that at a minimum meets the following salient characteristics in the Statement of Work under Section 7 for the VA West Los Angeles Healthcare System: ITEM DESCRITPTION UNIT QUANTITY 0001 Hercules Surgical Powered Table, Table and Table accessories. Model Hercules 6702 S EA 1 0002 Installation, Set up, Calibration and Testing JB 1 Include shipping cost on the items price. Vendor Requirements: Vendor shall provide delivery 30 days after receipt of order or sooner. Vendor shall quote all items under this requirement. Vendor shall provide standard warranty on the product and installationstarting from the first day of delivery. STATEMENT OF WORK FOR TIBOR RUBIN VA HOSPITAL MEDICAL CENTER, LONG BEACH, CA Objective: The contractor will purchase Powered Surgical Table and install in the Operation Room. The contractor is responsible to bring ladders, tools, testing equipment and software to install Powered Surgical Table at our locations on the VA Medical Center in Long Beach, Ca. This equipment will be installed to work with the current functional software Skytron system. Period of Performance: 30 days after receipt of order.� Location: LOCATION ADDRESS VA Long Beach, CA VA Bldg. s 126, 5th floor 5901 E. 7th Street Long Beach, Ca 90822�������������������������������� Product Description: The Powered table must be brand name (SKYTRON) or equal 6702 HERCULES POWERED SURGICAL TABLE Quantity ITEM DESCRITPTION ITEM # UNIT QUANTITY 0001 Powered Surgical Table 6702 EA 1 Salient Characteristics All salient characteristic requirements stated below must be met to accommodate the needs of the government: The table must be compatible with existing software at the VA Long Beach. This equipment will be installed to work with the current functional software system that is providing service to Patient Care at buildings 126, Operation Room. Must have top rotation allowing the surgeon or C-arm to remain at the foot end of the table Must have one-touch positioning functions to include return to level and bench chair Must have generous weight capacity for safety Must facilitate patient safety Must have removable back and leg sections for quick accessory attachment and positioning Must have a floor lock for emergency for patient care Must have a foot control connector and electrical panel Powered Table must have an emergency backup control Must have Patient Ready, once the table is locked.� What this means is once you put the table into the Operation Room and into the location of use, staff simply push the lock button on the pendant.� This will lock the table by lowering the 4 casters and self-level the table to assure the surgical staff will be ready to place the patient on the table and start the procedure. Installation The contractor is responsible to bring ladder, tools, testing equipment, and software to install Powered Surgical Table in bldg. s 126, Operation Room, 5th Floor. The POC will walk the contractor to location of the Powered Surgical Table s at the perspective buildings mentioned. Installation includes uncrating the table, set up, calibration, and function testing. Vendor shall provide relevant documentation and electronic copy of operators manual and parts and maintenance manual. Contractor is responsible for all clean up and taking trash out after installation. Inspection The VA POC shall ensure all delivery of supplies and installation is completed with final inspection by POC. Warranty 1-year Parts and Labor on Products 90 days on replacement parts, spare bulbs (Surgical Lights), spare pads (Surgical Table), Supplies, and accessory items The information identified above is intended to be descriptive, not restrictive and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/services that fulfill the required specifications. If you are interested and are capable of providing the sought-out supplies/services, please provide the requested information as well as the information indicated below. If you do, state how and what is altered; assembled; modified? (5) Does your company have an FSS contract with GSA or the NAC or are you a contract holder with NASA SEWP or any other federal contract? If so, please provide the contract type and number. (6) If you are an FSS GSA/NAC or NASA SEWP contract holder or other federal contract holder, are the referenced items/solutions available on your schedule/contract? (7) Please provide general pricing for your products/solutions for market research purposes.
