36C25520R0032 THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR A PROPOSAL OR QUOTE. THE VA IS NOT SEEKING PRICING OR OFFERS. GENERAL: This is a Request for Information (RFI)/Sources Sought Notice, issued in accordance with FAR 15.201(e), to conduct market research. This RFI/Sources Sought Notice is issued solely for information and planning purposes. It does not constitute either a Request for Quotation (RFQ) or a Request for Proposal (RFP); nor is it a promise to issue a RFQ/RFP in the future. This RFI/Sources Sought Notice does not commit the Government to contract for any supply or service whatsoever. The Department of Veterans Affairs (VA) is not seeking proposals at this time, and will not accept any unsolicited proposals. Responders, and not the Government, will pay for any costs incurred while responding to this RFI/Sources Sought Notice. Vendors who don t respond to this RFI/Sources Sought Notice may still participate in any future RFQ/RFP. Information submitted in response to this RFI/Sources Sought Notice is strictly voluntary and becomes the property of the Government. The Government may modify this announcement in the future, as information changes. The Government is not soliciting offers at this time. This is a RFI/Sources Sought Notice only, to gain knowledge of potential qualified sources. A solicitation is not currently available. Responses to this request will be considered for the purpose of determining how the requirement will be stated and how the procurement will be conducted. This RFI/Sources Sought Notice is not to be construed as a commitment by the Government, implied or otherwise, to issue a solicitation or to award a contract. No reimbursement will be made for any costs associated with providing information in response to this synopsis or any follow-up information requests. This RFI/Sources Sought Notice is a market research tool to help the Department of Veterans Affairs make acquisition decisions. The VA Heartland Network Contracting Office (NCO) 15 is looking for VETBIZ VIP verified SDVOSB concerns, all registered under the Product Service Code (PSC) of Q201 (Medical - General Health Care) and NAICS code of 339115 (Ophthalmic Goods Manufacturing), in SAM, that are capable of performing this service as described below. To be eligible, contractors must be registered in the SAM data base (https://www.sam.gov/SAM/) and verified in the VA s Vendor Information Pages (VIP) database (https://www.vip.vetbiz.va.gov/Home/Index) when an offer is submitted and at the time of contract award. DESCRIPTION OF SERVICES: To establish a single follow-on contract to VA255-16-C-0087, to provide all labor and materials necessary to provide eyeglasses and in-house (onsite at VA facilities) Optician services to beneficiaries of Veterans Integrated Service Network (VISN) 15 VA Medical Facilities. The contract will provide prescription eyeglasses to beneficiaries of the VISN 15 VA Healthcare systems which cannot be met with existing VA Medical Center prosthetics or logistics staff. The VISN 15 facilities to receive services are: Harry S. Truman VA Medical Center Columbia, MO VA Medical Center - Kansas City, MO Dwight D. Eisenhower - VA Medical Center Leavenworth, KS VA Medical Center Marion, IL John J. Pershing - VA Medical Center Poplar Bluff, MO Jefferson Barracks Division - VA Medical Center St. Louis, MO John Cochran Division - VA Medical Center St. Louis, MO Colmery O Neil - VA Medical Center Topeka, KS Robert J. Dole - VA Medical Center Wichita, KS Evansville Health Care Center Evansville IN Warrensburg VA Community Based Outpatient Clinic (CBOC) - Warrensburg, MO If the Government decides to acquire this service, the contract base period of performance (POP) would be for one (1) year, with four option periods of one (1) year each, plus an additional option period not to exceed six (6) months in duration.
