Description

The purpose of this sources sought is to gain knowledge of potential Small Business sources interested in Non-Personal Medical Service contract for Case Management, Behavioral Health Readiness Support, and Medical Credentialing. This notice is for planning purposes only. All interested parties should respond by submitting the following: (1) positive statement of intent to propose as a prime contractor, and (2) a completed and signed Source Sought Information Request Form (attached) and� (3) a listing of Past Performance completed by company during the past five(5) years, both for government and private industry. The type of performance, dollar value, contract number, location of performance, Point of Contact (POC), and POC email are to be included. (4)� A brief outline of resources, and key personnel that would be used to accomplish the contract. Information shall be provided via email to: sinamar.b.benauro.civ@mail.mil . Tennessee National Guard may issue a solicitation and award. All interested parties will provide this office, in writing, a notice stating their positive intention to submit a proposal as a prime contractor no later than February 3, 2020 at 2:00 PM central time.� Include the following in the subject line- Sources Sought W912L720R0003. �No facsimile responses will be honored.
