This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-02. The associated North American Industrial Classification System (NAICS) code for this procurement is 334510- Electromedical and Electrotherapeutic Apparatus Manufacturing, which has a corresponding business size of 1,250 employees. The Ralph H. Johnson VA Medical Center located at 109 Bee Street, Charleston, South Carolina, 29401 is seeking sources to provide an operating room (OR) video integration and recording system with upgraded camera heads for surgical procedures for six (6) surgical suites. This system will upgrade and replace the current video system from a HD system to a 4K system at the Ralph H. Johnson VA Medical Center. The system must provide monitors, light boxes, video boxes, camera heads, insufflators, speakers, mounting brackets, cabling and servers to develop a fully integrated 4k video system that can provide live streaming of the ORs and communicate with VA EMR (CPRS and Vista) for storage in addition to being capable of future expansion into the Cerner Electronic Medical Records (EMR), when applicable. The Contractor shall provide all necessary tools, interfaces, equipment/hardware, software, licenses, delivery, installation of new equipment (including new cabling and removal of old cabling) for a seamless transition to eliminate downtime, warranty, and training. Offerors are asked to provide a timeline for shipment, receipt, installation and testing of the equipment with their quote. Key Personnel: Key personnel will be identified in the proposal and shall be considered key personnel essential for the successful completion of the work performed under the contract. The contractor agrees that such personnel shall not be removed, diverted or replaced from the work without prior written approval of the Contracting Officer. The contractor shall submit written notice of proposed personnel changes to the Contracting Officer for approval at least thirty (30) business days in advance. Work at the Government site shall not take place on Federal holidays unless directed by the Contracting Officer (CO). Contractor will be required work outside normal business (Mon Fri 0800 -1700 ET) depending on degree of clinical impact. Coordination of this work will be communicated between the Contractor, COR, PM, and CO on an ad hoc basis, and decided on prior to work date. Any additional charges beyond the quoted amount must have prior approval by the CO/COR. Performance Period expected to begin in FY20 Q1, starting with contract award date and lasting no longer than 6 months days for the completion of development, delivery, installation, implementation, and testing. The services covered by this contract shall be furnished by the contractor as defined herein.� The contractor shall not be required, except in case of emergency, to furnish such services on a Federal Holidays. Appropriate notification of these access needs will be provided to the Government by the Contractor. Government shall provide adequate space in their closets for Contractor-provided equipment and Contractor-provided rack and cable management system. If there is a need for work to be done after hours, the Government will make arrangements for the Contractor to have access to the equipment. Project Kick-Off: A Project Kick-Off call shall be conducted within 14 calendar days of the contract award, as scheduled by designated representatives RHJ VAMC Healthcare Technology Management Service and Contractor. If there are no foreseen delays, the process shall be scheduled to begin within 30 calendar days after the Kick-Off call. A weekly or bi-weekly call schedule will be established between the project teams of the VA and the Contractor to coordinate action items, set project milestones, and provide prompt resolution to potential issues. The Contractor shall deliver a monthly Progress and Status Report covering all professional services completed during the reporting period and work planned for the subsequent reporting period.� The report shall also identify any problems that arose and a description of how the problems were resolved.� If problems have not been completely resolved, the Contractor shall provide an explanation including their plan and timeframe for resolving the issue. The COR shall monitor Contractor and Government performance against the Work Breakdown Structure (WBS), Quality Assurance Surveillance Plan (QASP), and project timeline (Gantt Chart) as established, and report any deviations. It is expected that the Contractor will keep in communication with VA accordingly so that issues that arise are transparent to both parties to prevent escalation of outstanding issues. This report shall be certified by the COR. Testing and Certification: Upon installation of each functional unit of the system, the Contractor shall notify the COR in writing that the system is ready for acceptance testing, and that it meets all requirements as specified. Contractor shall provide as-builts and system diagrams to the COR upon request. Upon receipt of the requested documents, RHJVAMC HTM staff will coordinate acceptance testing with the Contractor within seven (7) business days. Submission of this notification of system readiness shall be accomplished by the contractor prior to the beginning of the scheduled Government Acceptance Test. Any additional action that may arise to bring the signal up to the required standards will be completed at the vendor s expense. Government Acceptance Testing: Contractor shall perform the Government Acceptance Test in the presence of the COR for proof-of-performance. Upon completing the Government Acceptance Test successfully, Contractor shall document the success of this Government Acceptance Test on Contractor s company form which will be certified by the COR s signature as an evidence of Government Acceptance of the referenced system. A copy of this document shall be furnished to the COR and the Contracting Officer before Contractor s Representative at the end of each testing. Changes: Any changes to shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project.
