2-27-20- The purpose of this notice is to inform potential offerors that the DOD Safe Drop Off Request link for proposal have been�sent out today.� If you have not received a link�and should have, please send an email to Jackelin Guerrrero-Funes at jackelin.guerrero_funes@us.af.mil, Monique McLaughlin at monique.mclaughlin.2@us.af.mil, and 773ess.pk.proposal@us.af.mil. 2/26/20- The purpose of this notice is to request offerors to send an email to Jackelin Guerrrero-Funes at jackelin.guerrero_funes@us.af.mil, Monique McLaughlin at monique.mclaughlin.2@us.af.mil, and 773ess.pk.proposal@us.af.mil�to receive�the PWS mentioned in Amendment 0001 (Attachment 1 FAP Consolidated PWS 14 Feb 20).� 2/21/20- The purpose of this amendment is to: 1) Extend proposals due date to no later than 3:00 PM CST on 5 March 2020 2) Add DFARS clause 252.225-7974 - Representation Regarding Business Operations with the Maduro Regime (Deviation 2020-O0005). The remaining clauses within section I were carried over to Amendment 0001 however, remain unchanged. 3) Update CLIN�descriptions, Unit of Issue, and total Quantity for CLINs: 0001AA, 0001AB, 0001AD, 0002AA-AE, 0003 (description only), 0003AA-AB, 0003AD-AE, 0003AF, 0004 (description only), 0004AA-AF, 1002AB, 1002AC, 1002AE, 2002AB, 2002AC, 2002AE, 3002AB, 3002AC, 3002AE, 4002AB, 4002AC, and 4002AE. 4) Revise Attachment 1 PWS: sections: 1.2.3 (added non-offending caregivers), 2.4.3 (added telework), 3.1.6 (added cellphone/hotline), 3.1.7 (removed cellphone/hotline), and edited�PWS Attachment 4 header note. 5) Revise Attachment 19 52.212-1 Addendum: sections: A.1 (removed subcontractor restriction), A.2 (moved JV Letter to C.1.3), B. Table 2 (removed SAM), B.5 (revised font requirement and sub-volume organization), C.1.3 (added JV Letter), C.1.4 (added price guarantee statement), C.3 (removed predecessor company), C.3.2 (a) (revised font requirement), and 4.8 (b) (inserted PWS reference), and 4.8 (d) (added COLA calculation requirement) 6) Revise Attachment 20 52.212-2 Addendum: section A.3 (removed Subcontractor restriction). 7) Convert ITO Attachments 1,2,3,4 & 5 into a Word Document. 8) Remove informational instructions on ITO Attachment 1 FACTS Sheet. Please reference ITO Attachment 1 FACTS Sheet uploaded 28 Jan 2020 for instructional guidance. 9) Revise ITO Attachment 6 Pricing Model to update FTE Quantities for CONUS all years (FATM, FAIS, and DAVA); and update Annual Hours per FTE: CONUS Base Year (FATM,FAIS, DAVA, DAVA PC), USAFE Base Year (all labor categories), and PACAF Base Year (all labor categories). 10) Incorporate Attachment 25 Responses to formal RFP questions. The above are a majority of the changes made to the solicitation. However, all Offerors are responsible to read and comply with the amendment in its entirety. Potential Offerors are advised that an official RFP amendment 0001 will be forthcoming; anticipated within the next couple business days. 2/7/20- The purpose of this administrative amendment is to clarify to potential offerors that questions�are to�be submitted through the proposal box, or directly to the POCs. The question deadline remains 10 February 2020, 3:00 pm local San Antonio Time.�2/7/20 Post Revised 10:56 a.m. local San Antonio Time: Additionally, there is no specific template or format required for submission of questions. 2/4/20- The purpose of this administrative amendment is to state that all official Request for Proposal documents are dated 28, 29, and 31 January 2020. The Official RFP consists of the Solicitation FA805220R0002.� Supporting attachments are titled ""Attachment (number and�document name)"".� However, Attachment 19 (the Instructions to Offerors) has 8 attachments of its own titled ""ITO Attachment (Document Title)"". 1/31/20- The purpose of the administrative amendment is to unlock Column F in ITO Attachment 8 Cross Reference Matrix.� Beta.Sam functions differently between Internet Explorer and Chrome, it is suggested to utilize both browsers.� 1/29/20- The purpose of the administrative amendment is to include the omitted attachment 15 and to remove the watermarks off of ITO attachments 1 and 5 for the official RFP post. This is the official Request For Proposals (RFP) The RFP consists of Solicitation FA805220R0002, Attachments 1-24, and ITO Attachments 1-8; a�total of 33 documents. The FAP is a congressionally mandated program with The Office of the Secretary of Defense (OSD) oversite to provide prevention/treatment of Domestic Violence/Child Abuse (DV/CA) for military families IAW DoDI 6400.1, Family Advocacy Program, and DoDI 6400.06, Domestic Abuse Involving DoD Military and Certain Affiliated Personnel, to maximize readiness. FAP promotes public awareness/prevention of DV/CA and allows for restricted reports; provides assessment, rehabilitation, and treatment (including abuser intervention) for families, intimate partners, and victims of extra-familial child abuse; and provides victim advocacy services for military personnel and beneficiaries. FAP increases the competency/efficacy of families; preserves families where abuse occurred; and collaborates with state/local civilian social services, law enforcement, and medical agencies to prevent further harm. Located at AF/AF-led joint bases with command sponsored families.
