Description
This Request for Quote (RFQ) is for One (1) Non-Personal Service Contract for a Radiologist on ""As Needed Basis"" to provide uninterrupted patient care services to be performed in the Radiology Department at Northern Navajo Medical Center (NNMC) located in Shiprock, New Mexico. Estimated hours are identified in the appropriate Line Items above The Government does not guarantee the contractor to work the full number of hours identified above, hours are only estimated. Radiologist services shall be performed in accordance with the attached Performance Work Statement (PWS) and Quality Assurance Surveillance Plan (QASP). Period of Performance: Date of Award through December 31, 2020. The assignment are through the stated period of performance or until the position is filled with a permanent hire. In such instances, the provider will be released from contract assignment. Work Shift: The shifts hours are 8 hours from 8AM - 4:30pm Monday through Friday. While call back is unusual, the contractor may be required to remain beyond scheduled hours to complete daily dictations or to provide Radiology reading services should Telerad services become unavailable. The Government DOES NOT guarantee a specific shift to any individual(s), shift hours shall greatly depend on patient case load. The supervisor at IHS facility shall determine specific tour of duty. The contract shall work 100% of the contracted hours.� Because this is a long-term assignment, the Government understands that the contractor may require vacation or CME time. This will be allowed no more frequently than 1 week every 8 weeks and must be communicated to the Radiology department scheduler 60 days in advance of desired vacation or CME. Housing: No government housing available. Facility Location: Northern Navajo Medical Center is located in the beautiful high-desert climate of the Four Corners area - where the four states of Arizona, New Mexico, Colorado, and Utah adjoin. Northern Navajo Medical Center provides: care services including family medicine, internal medicine, mental health, physical therapy, optometry, dental, social services, pharmacy, x-ray, laboratory, public health nursing, diabetes education, health promotion/disease prevention and outpatient rehabilitation services along with benefits coordination. Recreation opportunities exist such as camping, fishing, hiking, biking, backpacking, snowshoeing, snowboarding, cross country skiing, and alpine skiing. Telluride, Durango, Moab, Canyonlands, Grand Gulch, the Colorado River, the San Juan River, several large lakes, and Colorado Wilderness areas are just short drives away. CONDITION OF EMPLOYMENT: Type of Citizenship: U.S. Citizenship is required. As a Radiologist, the contractor shall perform all examinations and imaging reads independently: Specifically the Radiologist must have proven competency in the interpretation of MRI, Mammography, CT (head, chest, abdomen, pelvis, angiogram), X-ray and all modalities of ultrasound including, but not limited to Lower Extremity venous Doppler studies, breast, endovaginal and transabdominal, level II obstetrical and pediatric hip. Background Investigation: All qualified referred candidates will undergo Fingerprinting process. Fingerprint Card: FBI FD-258 (provided by the Federal Government): Fingerprinting MUST be cleared and approved prior to contact assignment. Contract assignment(s) are conditional on a cleared background check. OPM requires Electronic Questionnaire for Investigation Processing (e-QIP) for registered nursing services exceeding One Hundred Twenty (120) days. Electronic Questionnaire for Investigation Processing (e-QIP): Executive Orders 10450, 12968, and HSPD-12 require a background investigation as a condition of employment. This investigation will be processed utilizing the US Office of Personnel Management (OPM) system. QUALIFICATION / EVALUATION: The Government reserves the right to issue a single award or multiple awards to any contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, Price and other Evaluation Factors will be considered: Past Performance and Qualifications. The Evaluation Factors and significant subfactors when combined are significantly more important than cost or price. As part of the evaluation process, candidates shall be interviewed to ensure their understanding of the PWS and to verify their qualifications to perform required services. The socio-economic status of an offer may also be considered, should one or more quotes represent the best value. Factor 1: Agency�s Past Performance Subfactor: Number of similar contracts Subfactor: Timeliness of providing candidate(s) Factor 2: Radiologist Qualifications Requirements Subfactor: Degree: Doctor of Medicine or Osteopathy from a school of Allopathic or Osteopathic ������������������� Medicine Subfactor: Graduation from a Residency in Radiology accredited by the Accreditation Council of ������������������� Graduate Medical Education Subfactor:� Current Basic Life Support (BLS) is mandatory Subfactor: Professional Development Subfactor: General Experience / Specific Experience Subfactor: Equipment (Refer to PWS 6.3) Subfactor: Health Requirements (Medical Evaluation / Immunization) Medical Evaluation: The contractor shall provide a fitness for duty certificate issued by a licensed physician to perform the proposed job without significant risk to personal health or the health and safety of others. Immunization: Contractor shall provide evidence of current immunization documentation for MMR (measles, Mumps, and Rubella), Immunity to Hepatitis B series, and History of chicken pox (varicella) disease or positive titer, Tetanus Diphtheria (Td) within the last 5 years. Documentation of receiving a TB Mantoux skin test (PPD) within the past 12 months with documentation of follow-up for positive test.� A signed declination of Hepatitis B vaccination series shall be accepted. A copy of the document shall be submitted to the Employee Nurse upon arrival at the hospital before providing service. Credentialing and Privileging:� All qualified referred candidates must meet the I.H.S. criteria for credentialing and privileging as outlined in the performance work statement. Specifically candidate must not have a history of license suspension or suspension/revocation of Medical Staff Privileges. Applicant must not have a history of arrest or criminal conviction for any reason. Applicant must readily provide proof of privilege competency and conform with Medical Staff Bylaws, Rules and Regulations. Factor 3: Radiologist Availability Subfactor: Upon fingerprinting clearance, Radiologist shall be available as soon as possible to start. Subfactor: Radiologist shall be flexible to allow for short term (13-weeks), long term (1-year) and, where feasible, include intermittent support to meet unplanned needs that develop on a frequent basis. Subfactor: The Contractor shall staff the Radiology Department. Factor 4: Professional Skills: Subfactor: Ability & knowledge with infection control Subfactor: Ability & knowledge to learn the different types of equipment. Subfactor: Compliance with Medical Dental Staff Bylaws Rules and Regulations. PROPOSAL SUBMISSION INSTRUCTIONS: 1. Quotation in the line items above. The Government will consider competitive rates upon negotiation. Hourly Rate shall be an All-Inclusive Hourly Rate, Meaning travel, per diem, lodging/housing, transportation, meals, and applicable taxes (i.e. federal, state and local). The Attach Wage Determination Service Contract Act applies. 2. Past Performance: Provide experience of comparable scope and complexity in providing services within the past year. 3. Qualifications: Provide complete candidate Profile (Curriculum vitae, updated resume, licensures / Board Certification 4. Availability of candidate(s). NOTE: Incomplete Submission(s) shall not be reviewed nor considered if submitted directly to the hospital unit managers. In addition, any profiles received after the RFQ closing date shall be accepted but shall not be subject for review.
