Page 1 (the SF1449) does not have a street address and provides an email address for Cynthia Grygier in box 9. Are we to submit our proposal in electronic format to Ms. Grygier? Is there currently an existing portal and if one is in use, will it be transitioned to the awarded contractor? An electronic portal isn t currently being utilized. The PWS did not mention bariatric (heavy duty) wheelchairs. Will the Contractor be notified in advance of any patients requiring bariatric wheelchairs to ensure availability of bariatric lap belts and extension belts? Yes, the contractor will be notified of bariatric patients. In the event of a significant and unexpected price increase in the cost of doing business (e.g. fuel increases to $5.00/gal), can the contractor expect that a supplemental cost invoice will be honored? This is a firm fixed price contract. Supplemental costs will not be reimbursed. End of Document
