Page 2 of 2 The Department of Veterans Affairs, Great Lakes Acquisition Center (GLAC) is searching for vendors with the capability of providing special mode transportation services to the beneficiaries of the Clement J. Zablocki VA Medical Center in Milwaukee, WI residing in eastern, southeastern, northeastern, south central, north central and central Wisconsin. Transports may periodically be required in all of Wisconsin, the Upper Peninsula of Michigan, eastern Minnesota, and northern Illinois. Transportation destinations may include VA and non-VA medical facilities, Veteran domiciles, and other locations. The primary VA Medical facilities under this contract include: Clement J. Zablocki VA Medical Center John H. Bradley (Appleton) CBOC 5000 W. National Ave. 10 Tri Park Way Milwaukee, WI Appleton, WI 54914 Union Grove CBOC Cleveland CBOC 21425 Spring St. 1205 North Ave. Union Grove, WI Cleveland, WI 53015 Milo C. Huemphner (Green Bay) CBOC 2851 University Ave. Green Bay, WI 54311 The Contractor shall furnish the necessary personnel and equipment to provide for twenty-four (24) hour 7 day a week wheelchair van and sedan transportation services to the beneficiaries of the above-listed locations. Approximately 70% of the transports will occur within Milwaukee County. The North American Industry Classification System (NAICS) code for this procurement is 485991. All interested, responsible sources must submit a capability statement by Thursday, April 2, 2020 to maureen.falaschi@va.gov to include the following: Company Experience in providing transportation services to medical patients, to include number of years and any examples of current contracts in place with government or commercial entities; Customer point of contact information for contracts listed in company experience along with a description of services provided under the contract (types of services provided, volume of trips per year, geographic service area covered; Size and type of fleet (sedans and vans rear loaders and side loaders) currently owned or leased by the company submitting the capability statement; Number of drivers currently employed by the company submitting the capability statement; Ability to provide immediate access and communication between customers and their personnel 24 hours a day, 7 days a week; Example(s) of quality control programs and procedures currently utilized; DUNS number, organization name, organization address, point of contact, business size under NAICS 485991, and socioeconomic status; Licensing for states and state requirements. This RFI is issued solely for information and planning purposes to determine availability of qualified vendors and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. Please note that this is not a request for proposal and the Government is seeking information for market research purposes only. The Government may or may not issue solicitation documents.
