Description
Pre-Solicitation Notice for notice of intent to award a sole source contract. This is not a request for quotes nor a solicitation for offers. All responsible sources may submit a capabilities statement for possible consideration in future procurements by the Government to Joshua.Gallien@va.gov no later than 2:30 PM EST, 30-April-2020 The purpose of this proposed contract is to establish a STRYKER In-Touch Beds / SECURE II Beds and Stretchers maintenance contract. A contract shall be awarded for a base year (12 months) with four (4) potential annual option years to be exercised at the discretion of the government. Location of facilities where work shall take place: West Haven VAMC Newington VA 950 Campbell Ave 555 Willard Ave West Haven, CT 06516 Newington, CT 06111 SCOPE OF WORK A. Replace and repair STRYKER In-Touch bed surfaces, (Iso-Librium / Expert ), ISO FLEX support surfaces, major board components with OEM parts. B. Replace and repair STRYKER Secure II ISO Flex support surfaces, major board components with OEM parts. C. Replace and repair STRYKER STRECHERS and support surfaces, major board components with OEM parts. D. The repairs will require the replacement of major failed components. The repairs will include labor, and miscellaneous materials to restore the full functionality of STRYKER beds. Noted are the major failed equipment and components that require replacement as follows. E. The repairs shall include a review of bed functionality test and user operation instructions. F. Contractor shall provide documentation with approved OEM manufactures service/safety check sheets. G. The repairs and replacement of bed surfaces shall include a 3 year or greater manufactures warranty. Preventive maintenance program shall be performed on date of repairs with available 1 year of warranty service. H. All OEM replacement and labor parts shall be Stryker approved (OEM parts) and performed by factory certified personnel. E. Salient characteristics regarding maintenance: Service technician shall be trained and certified by original equipment manufacturer Direct factory parts supplied by original equipment manufacturer shall be used 2 Hour maximum response time after service call is entered Turnaround time for repairs shall not exceed 72 hours after equipment is assessed by service technician CMS, DNV & Joint Commission documentation shall be provided Comprehensive and detailed preventative maintenance quality inspection procedures to be performed annually on contracted equipment Quality Inspection Procedures pertaining to annual Preventative Maintenance (PM) scheduled items below shall be inspected and documented throughout the annual PM assessment: All fasteners secure No tears, rips, holes, cracks, or other openings in mattress cover Brake mechanism operates properly Siderails move, latch and stow properly All functions on siderails working properly Manual CPR release working properly No cracks or splits in head or footboard All functions on footboard working properly Motion interrupt switches working properly Brake LED light located on footboard working properly Siderail switches working properly Bed status light bar LED s working properly Bed status side indicator LED s working properly Confirm head end control panel functionality Steer function operates properly Foot prop intact and working properly IV Pole intact and operating properly Scale calibrated properly Inspect siderail gas spring for oil leaks and replace if necessary Inspect fowler angel for accuracy Power cord and plug are free of damage Cables inspected to ensure none are worn, pinched, or frayed All electrical connections secure All grounds secure to the frame Leakage test conducted Apply grease to the bed grease points Check labels to ensure legibility, proper adherence and integrity PROCARE COVERAGE STRYKER PRODUCTS: STRYKER MODEL NUMBER Model Description QTY STRYKER MODEL NUMBER MODEL DESCRIPTION QTY 1015 M Series w/Big Wheel 8 1061 Gynnie OB/GYN 1 1115 Prime Big Wheel 12 1125 Prime Zoon 56 1460 Prime TC 34 2141 In Touch Zoon 38 Bed Pump BED / Air Pump 10 2971 In Touch Bed w Isolibrium Surface 43 3002 Secure II Bed 100 5700 Spirit Select Bed 20 Preventive Maintenance: *Includes parts, labor, travel *Includes 1 annual PM inspection *Includes unscheduled service & Joint Commissions Accreditation On Hospital Organization (JCAHO) equipment checklists. *Replacement parts do not include mattresses, and other Disposable or expendable parts STRYKER Corporation located at 3800 E Centre Ave, Portage, MI 49002 is the Original Equipment Manufacturer (OEM) for this STRYKER equipment. Due to their proprietary technology, they are the only vendor that will be able to service these machines as the Authorized Service Provider. Since the intent is to procure these services on a sole source basis, no solicitation will be posted to Contract Opportunities. The applicable NAICS code is 811219. The FSC/PSC is J065. The authority for applying the Simplified Procedures for Commercial Items of� FAR 13.5 is 41 U.S.C. 1901 and is implemented by for restricting competition on this procurement via FAR 13.106-1(b)(2).
