- Combined Synopsis-Solicitation for Commercial Items VHAPM Part 813.106 SAP: Soliciting Competition & Evaluation of Quotations SOP Page 3 of 5 Original Date: 10/12/17 Revision 02 Date: 10/15/19 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2020-06. This solicitation is set-aside for small business. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112, Surgical and Medical Instrument Manufacturing with a small business size standard of 1,000. The FSC/PSC is 6515. The Network Contracting Office (NCO 10), 3140 Governors Place Blvd, Suite 210, Kettering, Ohio 45409 is seeking to purchase Virtual Reality Hardware/Software. All interested companies shall provide quotations for the following: Supplies Line Item Description Quantity Unit of Measure Unit Price Total Price 0001 Hardware rental-12 months Android tablet 4 EA 0002 Hardware rental-12 months Hardware station 4 EA 0003 Platform software-12 months 4 EA The Louis Stokes Cleveland VA medical center is in need of virtual reality hardware/software to optimize the physical rehabilitation of the veterans of Northeast Ohio. This procurement will provide improved cognitive and physical functioning of Veterans post stroke, spinal cord injury and traumatic brain injuries. Virtual reality therapy (VRT), also known as virtual reality immersion therapy (VRIT), simulation for therapy (SFT), virtual reality exposure therapy (VRET), and computerized CBT (CCBT), is the use of virtual reality technology for psychological or recreational therapy and in affecting virtual rehabilitation. Patients receiving virtual reality therapy navigate through digitally created environments and complete specially designed tasks often tailored to treat a specific ailment. The vendor will be responsible with supplying the listed virtual reality systems and accessories. Delivery location is Cleveland VA Medical Center, Cleveland, OH, and shall be FOB destination. Delivery required within 30 days of contract award. The solicitation number is 36C25020Q0612 and is issued as a Request for Quote (RFQ), unless otherwise indicated herein. This requirement is posted as a small business set aside. Businesses submitting responses to this RFQ not listed and current in the System for Award Management (SAM) (https://www.sam.gov/) prior to award will not be considered, and price quotations received will not be evaluated. (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be the Lowest Priced Technically Acceptable offer. Evaluations will be conducted under FAR Part 13.106-1(e) and any subsequent award will be made to the offeror that provides the Lowest Price Technically Acceptable to the Government. Award shall be made to the contractor whose quote is the most advantageous to the government, considering technical capability, past performance, and price. The government will evaluate information based on the following evaluation criteria in no particular order: technical capability factor meeting or exceeding the requirement, past performance, and price. (b) A written notice of award or acceptance of an offer mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
