Salient Characteristics Amsco 400 20X20X38 Prevacuum Single Sliding Cabinet Electric 480V The AMSCO 400 SERIES 20 Small Steam Sterilizer features a high capacity chamber that has been validated to sterilize up to three 25lb instrument trays. Increased shelf clearance accommodates larger sets. 12 programmable cycles include Immediate Use Prevac 4+1, Gravity and Liquids. Cycle names are customizable. The chamber is constructed of solid 316L stainless steel and carries a 15 year warranty. The pressure vessel has a full jacket design to minimize the formation of excess condensation on the chamber interior. Steam activated gasket requires no lubrication and is warranted for 2 years. The automated vertical sliding door is foot pedal activated for hands-free operation. Enhanced display is located at ergonomic height and features large cycle countdown visible from 15 feet away. Ink on paper impact printer provides permanent cycle record. Utilities usage is minimized with multiple temperature sensors. System operation can be continuously monitored with STERIS Proconnect remote service monitoring. Integrated 35kW electric steam generator includes automated flush and drain function to ease maintenance requirements and extend generator life. AMSCO� 400 Small Steam Sterilizers 20"" x 20"" x 38"" Prevacuum Single Door Integral Steam Generator Standard configuration with a Venturi - single phase power only 480V Amsco 400 Series Single Door 20 X 20 X 38 Rack & Two Shelves, Single Door 52.211-6 Brand Name or Equal. As prescribed in 11.107(a), insert the following provision: Brand Name or Equal (Aug 1999) ����� (a) If an item in this solicitation is identified as brand name or equal, the purchase description reflects the characteristics and level of quality that will satisfy the Government s needs. The salient physical, functional, or performance characteristics that equal products must meet are specified in the solicitation. ����� (b) To be considered for award, offers of equal products, including equal products of the brand name manufacturer, must- ���������� (1) Meet the salient physical, functional, or performance characteristic specified in this solicitation; ���������� (2) Clearly identify the item by- ��������������� (i) Brand name, if any; and ��������������� (ii) Make or model number; ���������� (3) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and ���������� (4) Clearly describe any modifications the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modifications. ����� (c) The Contracting Officer will evaluate equal products on the basis of information furnished by the offeror or identified in the offer and reasonably available to the Contracting Officer. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. ����� (d) Unless the offeror clearly indicates in its offer that the product being offered is an equal product, the offeror shall provide the brand name product referenced in the solicitation. Background The Association for the Advancement of Medical Instrumentation states sterilizers should be installed according to the manufacturer s written IFU. Currently there are two (2) sterilizers in service located within the facility; that due to workload, are reaching lifecycle expectancy prematurely. SPS had a previous existing contract with STERIS. The goal is to acquire a contract to follow the ANSI/AAMI ST79:2010 guidelines; para 9.3. VISN 17 Instrument Sharpening and Repair deployment The sterilizer installation and removal contract will service the following site: Facility (VAMC/HCS) Address South Texas HCS (San Antonio) 8410 Data point Rm 110 San Antonio, TX 78229 GENERAL REQUIREMENTS Make: AMSCO Model: 400 Series 20 Small Steam Sterilizers Quantity: 2 ea Chamber Size: 20 W x 20 H x 38 D Unit Dimension: 74.5 H x 30 W x 45.63 D 3 Scope of Work The contractor shall perform onsite service which will include the following: Upon receipt of purchase order vendor will manufacture, configure and ship both sterilizers. Once the sterilizers arrive at STVHCS; vendor will uninstall the current sterilizers and make all necessary utility disconnections, remove and transport equipment to a predetermined location within on-site customer facility for pickup and disposal. Customer is responsible for decontamination and release of the equipment as non-hazardous. Equipment disposition from the staging point will be dependent upon customer selection of disposal option. If removed equipment is to become vendor property, it will be held on-site at the designated location for pickup by vendor representative within a reasonable time, not to exceed five (5) working days. Once the current sterilizers are removed; vendor installation includes all necessary labor (non-union) and materials required to uncrate, set-in-place and assemble sterilizers and components. Price is for normal working hours, Monday Friday 8 am -5 pm unless otherwise noted. Vendor also includes with its installation services an operational check-out test, any necessary adjustments and a demonstration of the equipment for appropriate facility personnel use. the software licenses This contract will not require services that involve connection of one or more contractor-owned IT devices (such as a laptop computer or remote connection from a contractor system) to a VA internal trusted (i.e., non-public) network. Subsequently, the C&A requirements do not apply, and a Security Accreditation Package is not required. QUALIFICATIONS To be determined responsible, a prospective contractor must: Install all equipment General Requirements The Contractor will coordinate with the Contracting Officer s Representative (COR) for the contract regarding the dates of on-site visits, and otherwise work with the COR to ensure compliance with the terms and conditions of the contract. PERIOD of PERFORMANCE Within 45-90 days of contract award Contract Administration Record keeping/monitoring procedures The COR will be responsible for ensuring that services are provided in accordance with the terms and conditions of the contract. security requirements 8.1 Information System Officer, Information Protection: The contractor will not have access to VA Desktop computers they will not have access to online resources belonging to the government. 8.2 Privacy Officer: The Contractor will not have access to protected Patient Health Information (PHI) nor will they have capability of accessing patient information during the services provided to the VA and if removal of equipment from the VA is required, any memory storage device will remain in VA control and will not be removed from VA custody. All research data available for Contractor analysis is de-identified. 8.3 Records Manager: a. Citations to pertinent laws, codes and regulations such as 44 U.S.C chapters 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228. b. Contractor shall treat all deliverables under the contract as the property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. c. Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. d. Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. e. Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract. f. The Government Agency owns the rights to all data/records produced as part of this contract. g. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data. h. Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.]. i. No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules. j. Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-contractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under, or relating to, this contract. The Contractor (and any sub-contractor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information. 8.4 Physical Security a. An ID badge will be issued by the VA Police which must be presented by all contractors at the VA entrance security check points. The issued badge is designated for official pick-up and deliveries of surgical instruments and trays. b. ID badge must be worn at all times above the waist, in front, with the face of the card visible. c. It is the responsibility of the contracting officer representative or designee to verify that the vendor has a temporary identification badge. If not, they are to be escorted to the VA Police and Security office. Once issued a temporary identification badge, the contractor will be escorted by the contracting officer representative or designee to SPS. d. Contractor s mobile unit/van shall be properly labeled with the company logo and registered with the VA Police Department and park in designated areas for service. e. Upon arrival to the Sterile Processing Service, the contractor will be required to contact contract officer representative (COR) and/or shift supervisor.
