- CUTOFF FOR QUESTIONS: 14 May 2020 1 P.M. CST The Department of Defense Military Treatment Facilities (MTF) in San Antonio, Texas, have a requirement for personal services as described in this PWS. This is a follow-on requirement for services currently being performed. The Contractor shall provide personal services for Endocrinology Diabetes Center of Excellence (EDCoE) referred to as the Health Care Provider (HCP) / Contractor Personnel at San Antonio, Texas Department of Defense Military Treatment Facilities (MTF). The contract personnel and HCP for personal services provide comprehensive diabetes management to any Tricare beneficiary that is referred to the clinic for care. The EDCoE mission and vision encompass a three-tiered approach: Diabetes management, outreach/education, and translational research. The management section include diabetes prevention through an intensive diabetes prevention program, diabetes education through an American Diabetes Association recognized education program, and intensive diabetes management (includes use of complex regimens, insulin pump, glucose sensors, comprehensive foot exams, vaccination programs, and specialized testing). The research section conducts translational research, investigator initiated research, and validation of new treatment models. The research section gains approval through the Institutional Review Board for all studies. The outreach/training section provides services and products that support any DoD MTF. Those services include management of the Diabetes Central website, Diabetes Champion Course (multi-day education course every 6 months), Diabetes ECHO (monthly provider webinar), Diabetes Webinar (monthly nurse webinar), as well as developing additional programs based on other MTF input. The contractor and HCP shall perform services and support compatible with the medical facility�s operating capacity and equipment.
