Description
SOLICITATION NOTICE OF TIERED EVALUATIONS This procurement is set-aside based on an order of priority as established in 38 U.S.C. 8127. TIERED EVALUATIONS INCLUDING SMALL BUSINESS CONCERNS: This solicitation is being issued as a tiered evaluation for SDVOSB concerns, or in the alternative, a tiered evaluation for VOSB concerns, or in the alternative, a set-aside for other small business concerns with HUBZone small business concerns and 8(a) participants having priority. If award cannot be made, the solicitation will be cancelled and the requirement resolicited. The Department of Veterans Affairs (VA), VISN 5 VAMC Martinsburg, WV provides services to Veterans beneficiaries in West Virginia, Virginia, Maryland, and Pennsylvania. The Contractor shall be responsible for providing Government-Supplied Durable Medical Equipment (DME) pickup, setup, installation, disassembly, repair, preventive maintenance, site assessment, disposal, storage and delivery services to all beneficiaries serviced by VAMC Martinsburg. The contractor shall provide transportation of equipment, equipment services, labor, supervision, patient education, safety management, infection control, as necessary for patients receiving DME services, as specified under this requirement. These services shall be required between the hours of 8:00 a.m. and 4:30 p.m., Monday through Friday, except in cases of emergency. The contractor shall be staffed and stocked to render services provided in accordance with all terms and conditions of this requirement for the duration of the performance period. Offerors should thoroughly review the specifications and be familiar with the area of coverage before submitting their proposal in order to be fully aware of the supplies and the services required. Failure to do so will not relieve the offeror from performing in accordance with the strict intent and meaning of the specifications without additional cost to the Government. Performance period is June 15, 2020 to June 14, 2021 with (4) one year ordering periods. The solicitation will be available approximately seven (27) calendar days from the date of publication of this announcement. The solicitation will be available at http://www.fedbizopps.gov . Amendments to the solicitation will also be posted on this website. All respondents must be registered in the SAM data base, see https://www.sam.gov to include the completion of Representations and Certifications and registering with Vets 4212 at https://www.dol.gov/vets/vets4212.htm . Offerors must be registered in VetBiz at http://vetbiz.gov/ All parties must obtain amendments to the solicitation through the web site. Hard copies will not be mailed. The contract will be awarded in accordance with the procedures specified in the solicitation. The solicitation will be issued as a Request for Proposal (RFP) conducted in accordance with FAR Part 13.5. This project is being solicited as a TOTAL 100% SMALL BUSINESS SET-ASIDE (SB). The North American Classification System (NAICS) Code for this procurement is 621610, with a small business size standard of $16.5 million annual receipts for the past three (3) years. The estimated magnitude of this project is between $100,000 to $900,000 . The solicitation package, amendments, and site visit minutes will be issued by way of Contract Opportunities website only. Solicitation Issue Date will be on or about April 30, 2020. All questions regarding this solicitation must be submitted in writing by email to Bradley.Davis3@va.gov . All questions pertaining to this solicitation must be received in writing. A fixed price task order will be awarded as a result of this solicitation based on the availability of funds. It is the responsibility of the contractor to frequently check the web site for all notices, amendments, etc. regarding this solicitation. The government will not be responsible for any notifications not sent or received by contractors regarding this solicitation.
