- THIS REQUEST FOR INFORMATION (RFI) 36C26220Q0796 -IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. THE SUBMISSION OF PRICING, CAPABILITIES FOR PLANNING PURPOSES, AND OTHER MARKET INFORMATION IS HIGHLY ENCOURAGED AND ALLOWED UNDER THIS RFI IN ACCORDANCE WITH (IAW) FAR 15.201(e). SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a source sought/RFI for market research only. The purpose of this source sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to NAICS 337214 (size standard is $1,000 Employees). Responses to this source sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this source sought, a solicitation announcement may be published. Responses to this source sought synopsis are not considered adequate responses for a solicitation announcement. The Department of Veterans Affairs (VA), VISN 22 Network Contracting Office, is seeking sources that can provide Guest and Conference Chairs Furniture that at a minimum meets the following salient characteristics in the Statement of Work for the VA San Diego Healthcare System: ITEM DESCRIPTION UNIT QUANTITY UNIT PRICE AMOUNT 1 Mid-Grade Guest Chair EA 400 � � OM Werksy Chair � High-Tensile Steel Frame � Seat & back fabric: Momentum, Silica Stride, Limestone � Shell: Palladium Grey Molded Polypropylene � � 2 Small Basic Conference Chair EA 300 � OM Werksy Chair � High-Tensile Steel Frame � Seat & back fabric: Momentum Row, Oasis � Shell: Palladium Grey Molded Polypropylene � � TOTAL � Include shipping cost on the items price. Vendor Requirements: Period of Performance is 90 Days After Receipt of Order (ARO). Vendor shall quote all items under this requirement. Vendor shall provide standard warranty on the product starting from the first day of delivery. Vendor shall meet all Salient Characteristics in the Statement of Work. STATEMENT OF WORK FOR VETERAN AFFAIRS GREATER LOS ANGELES HEALTHCARE SYSTEM Introduction This statement of work (SOW) describes the requirement for the furniture upgrade for Mental Health Department Psychology patients and staff. This is for the replacement of any chairs that are worn, soiled, broken and beyond warranty and repair. Scope of Work This project consists to deliver the following: 400 Werksy Patient/Guest Chairs and 300 Werksy Hoteling Chairs. The guest chairs will be replacing any patient chairs throughout the Domiciliary buildings including offices, group rooms and multi-purpose rooms. The hoteling chairs will be placed in staff conference/meeting rooms. General Requirements Vendor shall provide to Greater Los Angeles Healthcare System with new Werksy Guest Chairs & Werksy Hoteling Chairs. The Werksy chairs meet the needs of this Mental Health facility and are already installed and well received at a GLA outpatient clinic at the Sepulveda Ambulatory Care Center. Tasks/Requirements The contractor shall be responsible for providing all sample materials (Fabric or Finish Materials) for awarded product (2-Day Delivery) as requested by VA Interior Designer. The product must not be placed into production without clearance from the VA Point of Contact (POC). The contractor shall track manufacturing schedule and notify VA Interior Designer with updates of estimated completion date by email. The contractor shall provide up to 30 days free storage [30 days standard] following the 60-day period of performance if requested by designer. The Government maintains the option to modify the normal work week, days and hours, as necessary for the accomplishment of VHA mission. Mandatory Tasks The vendor shall coordinate and provide project management of all products and services to order, ship, and deliver new furniture from the manufacturer until final acceptance by the VA POC. The vendor shall provide maintenance manuals for all products specified. The vendor shall provide and be responsible for the technical assistance, development and generation for final specifications. Period of Performance 90 days after receipt of order or sooner. Items will be Drop Ship only. Duty hours will be 8:00am-12:00pm. The Government recognized US holidays are: New Year s Day, Martin Luther King, Jr. Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. If the holiday falls on Sunday, it is observed on Monday. Inspection and Acceptance Upon acceptance, the interior designer shall: Inspect the delivery and sign off on any and all missing or damaged products in the form of a delivery manifest, vendor provided Ensure all delivery of merchandise is completed and received satisfactorily prior to acceptance. Vendor must provide furnishings, and these shall have smooth finishes with no hazardous projections, sharp corners, or detail, which can be hazardous and cause personal injury or damage to clothing Vendor shall provide assembled chairs. Vendor shall be responsible for any damaged products upon delivery and all liability actions or suits when the vendor is at fault or negligent in the performance period. The selected vendor shall furnish all items as described in the statement of work. Government Responsibilities GLA Interior Design will coordinate delivery date and time with selected vendor. The Government will provide the name of the VA POC at the time of contract award. Warranty All workmanship and furniture quality shall be covered under warranty by the selected vendor for a Minimum of 10 years. Please see salient characteristics for any further warranty requirements on 11.2.10. Installation No installation required for this procurement.
