Description
BOISE VA MEDICAL CENTER STATEMENT OF WORK March 31, 2020 1. Contract Title. Full Service Parts and Maintenance agreement for: 2 Sterrad NX Units, S/N: 33101431 and S/N: 33120631; 2 Evotech Units, S/N: 5041160080 and S/N: 5041160081 in SPS. 2. Background. Biomedical Department does not have the necessary specialized tools and training to perform maintenance and repair on the Sterrad NX and Evotech Units. 3. Scope. Contractor shall furnish all supervision, labor, travel, equipment, parts, and supplies to furnish preventative maintenance services and corrective / repair services for 2 Sterrad NX Units, S/N: 33101431 and S/N: 33120631; 2 Evotech Units, S/N: 5041160080 and S/N: 5041160081 located at the Boise VA Medical Center (BVAMC) SPS. Service will be provided between 8:00am to 5:00pm, Monday through Friday, excluding holidays. The system does not contain patient identifiable information and is not connected to the VA s Computer Network and the Vendor does not have the ability to log into the system remotely. The Vendor shall comply with the requirements of VHA policies pertaining to sensitive information and sensitive systems. 4. Specific Tasks. Contractor shall provide a copy of all Manufacturer specified scheduled maintenance as published in the Manufacturer s Service Manual to the BVAMC assigned POC. Contractor shall perform all Manufacturer specified scheduled maintenance as published in the Manufacturer s Service Manual. Scheduled Maintenance shall be performed at the Manufacturer s recommended frequency as published in the Manufacturer s Service Manual during the annual contract period. Contractor shall furnish all supervision, labor, travel, equipment, parts, software support with upgrades, and supplies to accomplish the preventative maintenance services, and corrective call back services during the annual contract period. Contractor shall be escorted by a VA employee and all work will be observed and monitored while performing work at the BVAMC. Contractor shall adhere to all VA Policies pertaining to sensitive information and sensitive systems while performing work on the Sterrad NX and Evotech Systems. OEM parts shall be used for all maintenance and repair services. Contractor shall provide a copy of the Service Report, either by email or by fax, no later than 5 work days after the completion of the scheduled maintenance or repair service. The Service Report shall contain the following information: Contract purchase order number Date service initiated Date completed Equipment make and unique identification number/code Description of original complaint Problems found Part/quantities installed All work performed fully described 5. Performance Monitoring Performance will be monitored by periodic inspections and observation of work. Work will be observed by SPS Staff, Contracting Officer s POC, or designee, during performance of scheduled maintenance service and inspections and/or during repair service. Equipment operators will be contacted to report on contractor performance when POC, or designee, is not available. 6. Security Requirements. No Security Requirements. Units are not in a Patient Care Area, are not in a Sensitive Area, Stores no PII or PHI, and is not connected to the network. As directed in 6500.6, Appendix A - The C&A requirements do not apply, and a Security Accreditation Package is not required. 7. Risk Control. The Contractor shall obtain all necessary licenses and/or permits required to perform this work. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to any contractor representative, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by the contractor s fault or negligence. 8. Place of Performance. All service will be performed at the BVAMC in SPS. 9. Period of Performance. Base year plus one option year. 4/5/20-4/4/21 (Est. Base year) 4/5/21-4/4/22 (Est. Option year 1)
