- THIS REQUEST FOR INFORMATION (RFI) 36C26220Q0813 -IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. THE SUBMISSION OF PRICING, CAPABILITIES FOR PLANNING PURPOSES, AND OTHER MARKET INFORMATION IS HIGHLY ENCOURAGED AND ALLOWED UNDER THIS RFI IN ACCORDANCE WITH (IAW) FAR 15.201(e). SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a source sought/RFI for market research only. The purpose of this source sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to NAICS 339112 (size standard is $1,000 Employees). Responses to this source sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this source sought, a solicitation announcement may be published. Responses to this source sought synopsis are not considered adequate responses for a solicitation announcement. Vendor Requirements: Period of Performance is 2 Months after receipt of order, this PoP includes delivering and assembling of the Slit Lamps. Vendor shall quote all items under this requirement. Vendor shall provide standard warranty on the product starting from the first day of delivery. Vendor shall meet all Salient Characteristics in the Statement of Work. STATEMENT OF WORK FOR SAN DIEGO VA HEALTHCARE SYSTEM, SAN DIEGO, CA Introduction The purpose of this statement of work (SOW) is to outline the tasks and responsibilities of the successful offeror in relation to providing a LED Slit Lamp imaging system. Background The San Diego VA Healthcare Hospital at 3350 La Jolla Village Dr. La Jolla, CA 92161 is part of the Desert Pacific Healthcare Network (VISN 22), and is one of the most diversified tertiary health care providers in the VA. It is a teaching hospital, providing a full range of patient services, with state-of-the-art technology as well as education and research. It currently needs eye imaging services in the Emergency Department (ED) at the recommendation of police services. Currently patients are sent up to the 2nd floor after hours to the Eye clinic for such services, this however presents a security risk for our providers. The solution would be to keep the patient in the ED for the afterhours procedure where security is already represented. Scope This procurement is to provide our Eye clinic with a new LED Slit Lamp imaging system. The LED Slit Lamp and imaging system requested, has a variety of features that allows the provider to evaluate the eye and to capture images of eye pathology for documentation. Currently we lack this ability in the Emergency Department, and this system would satisfy that requirement. No training is needed, as all our current providers are trained on this system. We currently own and operate several of them in the eye clinic. General Requirements Vendor shall provide to San Diego VA Hospital, A NEW LED Slit Lamp and Imaging Systems. The LED Slit Lamp and Imaging System is used to evaluate the eye and to capture images of eye pathology for documentation. Mandatory Tasks The Vendor shall ship A LED Slit Lamp and Imaging Systems to the San Diego VA Hospital. Once received at San Diego VA Hospital, the vendor shall physically assist Biomed in assembling the LED Slit Lamp and Imaging System. The VA POC will contact the vendor to coordinate the assembling of the Slit Lamps once received. Period of Performance/Acceptance: The Period of Performance (PoP) is 2 Months after receipt of order, this PoP includes delivering and assembling of the Slit Lamps. The slit lamp must be mounted on a table that is powered with 120VAC input. The Slit Lamp must be low voltage by converting 120VAC to DC voltage. This is commercial standard for the U.S. Slit lamp imaging system must be fully integrated/compatible with existing equipment, the Haag-Streit Slit Lamp with existing stereo variator. Slit lamp imaging system must have a history trigger burst of 30 frames or more. Slit lamp imaging system must have a Depth of field focus control. Slit lamp imaging system must have an adjustable aperture setting. Slit lamp imaging system must have fully integrated controls easily reachable from our existing Haag Streit BQ-900 Slit Lamp joystick for ease of photographer photo composition and capture. Vendor Responsibilities The vendor shall be responsible the delivery of this product. The vendor is responsible for delivery and assembling of the Slit Lamps. Warranty Warranty for 1 year which includes the replacement of defective parts and coincidental labor.
