- THIS REQUEST FOR INFORMATION (RFI/SOURCES SOUGHT IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES AND DOES NOT CONSTITUTE A SOLICITATION THE SUBMISSION OF PRICING, CAPABILITIES FOR PLANNING PURPOSES AND OTHER MARKET INFORMATION IS HIGHLY ENCOURGAGED AND ALLOWED UNDER THIS RFI IN ACCORDANCE WITH FAR15.201(e). DISCLAIMER: This RFI is issued solely for information and planning purposes and does not constitute a solicitation. Neither unsolicited proposals nor any other kind of offers will be considered in response to this RFI. IAW Far 15.201(e), responses to this notice are not offers and will not be accepted by the government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. All information received in response to this RFI that is marked Proprietary will be handled accordingly. Responses to the RFI will not be returned. At this time, questions concerning the composition and requirements for a future RFP will not be entertained. SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a sources sought/RFI only. The purpose of this sources sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to NAICS 811219 (size standard of 20.5 million). Responses to the Sources Sought will be used by the government to make appropriate acquisition decisions. After review of the responses to this sources sought, a solicitation announcement may be published. Responses to this Sources Sought synopsis are not considered adequate responses for a solicitation announcement. The Department of Veterans Affairs (VA), NCO 05 Network Contracting Office, is seeking sources that can provide maintenance of ScriptPro products and systems being utilized by the Pharmacy at the Washington DC VA Medical Center. The information identified is intended to be descriptive, not restrictive and to indicate the quality of supplies/services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/services that fulfill the required specifications. If you are interested and capable of providing the sought services/supplies, then please provide the requested information as well as the information indicated below. Responses to this notice should include: Company name, address, and point of contact Any small business designation as certified by the Small Business Administration, size of business pursuant to the RFI. A thorough description of experience managing requirements of this nature. 1). Please indicate the size status and representations of your business, such as, but not limited to: SDVOSB, VOSB, Hub-zone, Woman owned Small Business (WOSB), etc. 2). Is your company considered to be a small business under the NAICS code identified for this RFI? 3). Are you a manufacturer, distributor, or an equivalent solution to the items being referenced? 4). If you are a large business, do you have any designated distributors? If so, please provide the company names, telephone numbers, points of contact and size status. 5). If you intend to sub-contract any work of the contract, what portion of the total cost will be self-performed? Please provide estimated detailed percentage breakdowns related to sub-contracted work and completion of the job. 6). Does your company have an FSS contract with GSA or the NAC, or a NSA SEWP or any other federal contract? Please proved the contract number and any items/solutions available on your schedule/contract. 7). General pricing of your products, solution to the calibration of the listed audiology equipment is encouraged. Pricing will be used for market research only. It will not be used to evaluate for any award. 8). Please submit your capabilities and any information needed to establish capabilities for planning purposes. 9). Please provide your DUNS number. Responses to this Sources Sought notice are not a request to be added to a prospective bidders list or to receive a copy of the solicitation. GENERAL INFORMATION Title of Project: Maintenance of ScriptPro products and systems being utilized by the Pharmacy at the Washington DC VA Medical Center (DCVAMC). STATEMENT OF WORK SCOPE OF WORK: The Contractor shall provide all labor, personnel, equipment, tools, materials, supervision and other items and services necessary to perform service and maintenance to support the ScriptPro products, listed below, at the Washington DC Veterans Affairs Medical Center, 50 Irving Street NW, Washington, DC 20422. 1. EQUIPMENT: The contractor shall provide all tools and equipment necessary to properly and safely perform the services set forth within this statement of work. This shall include, but is not limited, to all Personal Protective Equipment (PPE) which may be required for the work to be completed in a safe manner. 2. AREAS TO BE SERVICED: Washington DC VA Medical Center 50 Irving Street NW Washington, DC 20422 3. TASK FREQUENCY AND INSTRUCTIONS: The contractor will provide 24x7x365 Help Desk support to address questions. Support is provided via telephone and remote access to installed systems. The contractor will be reached by calling 1-800- number. The contractor shall provide on-site field support for diagnostic and repair services. The services will be provided by qualified personal. The technicians will arrive during the pharmacy s standard hours of operation. The minimum standard response time for dispatch will be no later than the following workday after notification of equipment malfunction. All parts, labor, and travel to perform service and repairs are included in the monthly support fee. Parts requiring replacement to maintain the equipment in accordance with the contractor s specifications will be replaced on an exchange basis and may be either new or rebuilt parts. The contractor will install all parts needed for service and repair except for Customer Replaceable Parts that can be replaced by pharmacy staff with reasonable effort. ScriptPro will provide a list of Customer Replaceable Parts upon request. The contractor will provide preventative maintenance services. The contractor will schedule preventative maintenance to be performed on-site or remotely. Service reports for any and all work done are required to be submitted to the Biomedical Engineering Section within three business days of completion of the work. 4. SPECIAL WORK REQUIREMENTS: None. 6. ATTACHMENTS: None. 8. COMPLIANCE WITH OSHA BLOODBORNE PATHOGENS STANDARD: The contractor shall comply with the Federal OSHA Bloodborne Pathogens Standard. The contractor shall: A. Have methods by which all employees are educated as to risks associated with bloodborne pathogens. B. Have policies and procedures which reduce the risk of employee exposure to bloodborne pathogens. C. Have mechanisms for employee counseling and treatment following exposure to bloodborne pathogens. D. Provide appropriate personal protective equipment/clothing such as gloves, gowns, masks, protective eyewear, mouthpieces for the employee during performance of the contract.
