Description
DRAFT PERFORMANCE WORK STATEMENT The contractor shall furnish on-site commercial secured collection and destruction of confidential documents and patient privacy information for the following locations of the VA Gulf Coast Veterans Health Care System (VAGCVHCS): (1) Biloxi Division, 400 Veterans Avenue, Biloxi, MS 39531; (2) Mobile Outpatient Clinic, 4444 Demetropolis Road, Mobile, AL 36619; (3) Joint Ambulatory Care Center, 790 Veterans Way, Pensacola, FL 32507 (4) Eglin Outpatient Clinic, 100 Veterans Way, Eglin AFB, FL 32542; (5) Panama City Beach VA Clinic, 2600 Veterans Way, Panama City Beach, FL 3240; and, (6) Panama City Beach VA Clinic West, 140 Richard Jackson Blvd. Panama City Beach, FL 32407. The contractor shall be required to provide all personnel, supervision, and other services necessary to perform the work required by the contract. Normal business hours are 8:00 a.m. to 4:30 p.m., Monday through Friday. The Contractor shall perform to the standards expressed in the contract. All shredding shall take place at Government facilities or grounds and shall be witnessed by a VA employee authorized to witness destruction sensitive VA material. The contractor is responsible for maintaining liability insurance for the duration of the contract and for following all pertinent Federal and state regulations regarding transportation and destruction of classified medical documentation. The contractor will ensure that access to Personally Identifiable Information (PII) and Protected Health Information (PHI) is limited to authorized personnel only and kept in strict confidentiality. In accordance with provisions of the Privacy Act and HIPAA, criminal penalties may result for knowingly and willfully disclosing information of this type. Shredding service is to be provided on site. Consoles are to be emptied and new bags installed in each console by contractor personnel upon servicing. Contractor will go to locations of consoles, remove materials from the locked containers and transfer from locked transport containers then to contractor s truck where materials are then immediately shredded into unrecognizable bits. Contractor will remove material secured in the locked containers only. The collection of material outside the locked containers is prohibited. Contractor will practice reasonable safeguards while collecting console materials containing PHI/PII ensuring the bins while opened/unlocked are not left unattended. Should the Contractor be approached by an individual(s) requesting entry into the container and/or the container bags, the Contractor will advise those individuals that entry or retrieving documentation is not authorized and they must contact the Privacy Officer. The Contractor s truck/equipment shall be locked and secured during the collection process of console material and/or while unattended. Contractor s truck/equipment shall produce unreadable material. Shredder will be of such capacity that limited quantities of incidentals such as paper clips, staples, rubber bands, CDs, patient plastic armbands and other types of paper can be shredded. Contractor shall maintain shredding truck/equipment and surrounding area free of debris, oil and fluids during the shredding process. Only authorized contractor staff allowed in the shredding truck/equipment and within the surrounding area during the destruction process. The Contractor will immediately advise the Privacy Officer and/or VA Police of unauthorized individuals attempting to enter the shredding truck/equipment or surrounding area during the destruction process. Shredding area should be kept clean and organized to prevent the possibility of losing un-shredded documents. Please annotate method of final destruction on bid. Contractor shall provide secure lockable containers (consoles) to collect and store materials to be shredded until such time that the shredding of the materials occurs. The consoles are to be provided, maintained and replaced at no charge to the VA. Contractor will provide console keys to the COR for issuance to VA employees and will maintain a current list of key assignments. Consoles will be installed in specific locations by contractor.
