Description
The Government contemplates awarding of an indefinite-delivery, requirements-type contract with firm-fixed session resulting from the future solicitation. The contract period shall consists of a 12 month base period with four (4) 12 month option periods for renewal. It is anticipated that eight one-hour sessions will be needed three times per year. One session is defined as one hour in length. Sessions will typically be scheduled every other month between the months of April and September. Sessions will be scheduled between the hours of 7:30 a.m. and 4:00 p.m., Monday through Friday, excluding Federal Holidays. The contractor will be notified in advance of the requested date of service and number of inmates to be examined. For further description of the services, read the attached Statement of Work (SOW). Services include, but are not limited to, � Evaluation of patients are referred by Health Services staff. � Perform digital breast screening mammograms. � Provide professional interpretations. Fees for professional services shall be included in the exam price. Reports must be typewritten of computerized format. Reports must be received within seven days of service, unless abnormalities are noted. FCI Waseca Health Services must be notified of abnormal reports within 48 hours of service. The contractor shall provide a copy of the mammogram image on CD for each mammogram performed. The mobile unit�s digital imaging capabilities shall allow the operator to readily complete approximately 25 patients per session. The number of patients requiring services will determine the length of session. The contractor will be responsible for equipment transportation, delivery and set up at facility, tractors, trailers, drivers, complete maintenance/service coverage on all equipment, training and staffing insurance, site planning and consultation, marketing, support, etc. For further description of the services, read the attached Statement of Work (SOW).
