- April 9, 2020 STATMENT OF WORK Sewer Maintenance 1) Introduction: a. The Main sewer line and three separate underground storage tanks need to be maintained periodically throughout the year. b. This contract Period of Performance (PoP) will be a base year with an additional four (4) option years. 2) Objectives: a. Clean and inspect sewer main and remove liquids and solids from three 2000 gallon tanks to ensure they continue to function properly. 3) Statement of Work: a. Performed quarterly- Remove buildup of foreign matter from sewer main. Visually inspect inside of pipe for any deficiencies. b. Performed semi-annually-Pump three underground holding tanks and remove all foreign matter from the bottom. Inspect the integrity of each tank to ensure it is functioning properly. 4) Billing: a. Contractor will submit invoices quarterly. 5) Working hours: a. Work will be performed from Monday through Friday 6:00am till 4:00pm (with the exception of federal holidays). Weekend and Holiday hours may be approved on a case by case basis. b. Observed Federal Holidays: Columbus Day Veteran s Day Thanksgiving Day Christmas Day, New Year s Day Martin Luther King Birthday Washington s Birthday Memorial Day Independence Day Labor Day 6) Submittals: a. Contractor will provide inspections documentation to the COR or Plumbing Shop Supervisor with in 10 working days of performing services. 7) Safety: a. Contractor is responsible for maintaining a safe working environment for their employees as well as the MVAHACS Staff. Work areas will be kept cleaned throughout the day and all trash and demolished materials properly disposed of daily. 8) Security: a. Contractors and employees are required to obtain a vendor s badge, and have it displayed on their outer garments at all times. 9) Parking/Storage: a. Parking is available in parking lots 11, 43A and 43B for vehicles (automobiles and Pickup trucks). Larger vehicles, equipment, job site trailers or storage containers must utilize parking lot 43A only. Contractor s contact information (name and phone number) is required to be displayed on all vehicles, equipment, trailers or containers that are stored overnight at the MVAHCS. Dropping off tools and materials at the loading dock is permitted but no parking is allowed in the loading dock area. Unattended vehicles in unauthorized parking areas will be ticketed by MVAHCS Police. END
